摘要
目的分析D2-40在食管早期鳞状细胞癌的诊断意义。方法选择医院2018年1月至2020年12月诊治的早期食管鳞癌患者79例。基础和病理信息:使用问卷调查法收集患者年龄(≤60岁、>60岁)、性别(男性、女性)。查阅患者病案信息,统计患者肿瘤分化(高、中、低)、肿瘤直径(≤36mm、>36mm)、淋巴结转移(有、无)、浸润深度(黏膜内癌、黏膜下癌)。检查D2-40表达情况。结果D2-40阳性例数有32例,阴性47例,阳性率为40.51%。D2-40阳性表达在肿瘤分化、肿瘤直径、淋巴结转移和浸润深度上差异比较有统计学意义(P<0.05)。D2-40高阳性率主要在低分化、肿瘤直径>36mm、淋巴结转移、浸润至黏膜下癌。Logistic结果显示肿瘤直径、淋巴结转移和浸润深度是D2-40阳性表达的危险因素(P<0.05)。结论D2-40表达情况能够预测食管早期鳞状细胞癌浸润深度、肿瘤直径和淋巴结转移情况,建议密切关注。
Objective To investigate the diagnostic value of D2⁃40 in early esophageal squamous cell carcinoma.Methods Seventy⁃nine patients with early esophageal squamous cell carcinoma in our hospital from January 2018 to December 2020 were enrolled.Basic and pathological data were collected using questionnaire including age(≤60 years or>60 years)and gender(male or female),and the clinical data including tumor differentiation(high,medium or low),tumor diameter(≤36mm or>36mm),lymph node metastasis(present or absent)and depth of infiltration(intramucosal or submucosal carcinoma)were collected,meantime,the expression levels of D2⁃40 was detected.Results D2⁃40 was positive in 32 cases and negative in 47 cases,with a positive rate of 40.51%.The positive expression of D2⁃40 was significantly dif⁃ferent among patients with different status of tumor differentiation,tumor diameter,lymph node metastasis and invasion depth(P<0.05).Patients with high positive rate of D2⁃40 were characterized by poor differentiation,tumor diameter>36mm,lymph node metastasis,and submucosal infiltration.Logistic analysis showed that tumor diameter,lymph node metastasis and depth of invasion were the risk factors of D2⁃40 positive expression(P<0.05).Conclusion The expression of D2⁃40 can predict the depth of invasion,tumor diameter and lymph node metastasis of early esophageal squamous cell carcinoma.
