刻意练习法在乡医三年制消化科临床实习中的应用探索

The application and exploration of deliberate practice in the threeyear clinical practice of gastroenterology department in rural medicine

摘要 目的研究乡医三年制医学生在消化科临床实习过程中应用刻意练习法学习问诊和查体,对比传统教学方法有无差异。方法选取首都医科大学延庆教学医院2018级、2019级乡医消化内科40名实习生为研究对象,将实习生根据实习顺序进行平均分组,两组根据统一的教学大纲进行消化系统常见疾病的问诊及腹部体格检查,对照组实习生接受传统教学模式,实验组实习生应用刻意练习教学模式,比较两组实习生的考核成绩、教学效果及教学满意度。结果实验组实习生总考核成绩显著高于对照组,两组之间相比,有统计学意义(P<0.05);实验组实习生增加了学习的兴趣、提高了学习的积极性,更加喜欢该种教学模式,教学满意度较对照组更高(95.00%vs85.00%),两组之间相比,有统计学意义(P<0.05)。结论刻意练习法应用在临床实习过程中可有效提高实习生考核成绩及满意度,值得在内科临床教学中应用推广。 Objective To study the application of deliberate practice method in learning inquiry and physical examination of three-year rural medical students in the process of gastroenterology clinical practice,and to compare with traditional teaching methods.Method Forty interns in the Department of Gastroenterology of Township Hospital of our hospital,grade 2018 and grade 2019,were selected as the research objects,and the interns were evenly divided according to the order of practice.The two groups were subjected to the inquiry of common diseases of digestive system and abdominal physical examination according to the uniform teaching syllabus.The interns in the control group received the traditional teaching mode,while the interns in the experimental group received the deliberate practice teaching mode.The assessment results,teaching effect and teaching satisfaction of the two groups of interns were compared.Results The total assessment scores of interns in the experimental group were significantly higher than those in the control group(P<0.05).Interns in the experimental group increased their interest in learning,improved their enthusiasm in learning,preferred this kind of teaching mode,and their teaching satisfaction was higher than that in the control group(95.00%vs85.00%),compared with the two groups(P<0.05).Conelusion The application of deliberate practice can effectively improve the evaluation results and satisfaction of interns in the process of clinical practice,and it is worthy of application and promotion in internal medicine clinical teaching.
