护士主导的常态化全方位管理模式在泌尿外科护理中断事件中的应用

Application of nurse led normalization and all-around management mode in urinary surgery nursing interruption

目的探讨护士主导的常态化全方位管理模式在泌尿外科护理中断事件(nursing interruptions,NI)中的应用效果。方法成立常态化全方位管理团队,收集护理中断事件资料,分析原因,制定并实施常态化全方位管理方案管理泌尿外科护理中断事件,统计护理中断事件的来源、类型、发生频率,比较干预前后中断事件的结局、中断事件持续时间。结果2021年10月-2022年9月共观察510h,发生4122次NI,平均8.08次/h。干预后中断事件发生频率明显低于干预前,P<0.05,差异有统计学意义。护理中断事件主要来源于患者家属(16.38%)、患者(15.48%)、护士同事(14.87%)、环境(13.05%)。护理中断事件类型构成:侵扰型(47.65%)、分心型(25.62%)、毁损型(15.02%)及冲突型(11.72%)。与干预前相比,干预后护理中断事件消极型结局的占比明显减少,P<0.05,差异有统计学意义。干预后中断事件持续时间明显短于干预前,P<0.05,差异有统计学意义。结论护士主导的常态化全方位管理模式能有效减少泌尿外科护理中断事件,改善中断事件结局,缩短中断事件持续时间。

Objective To explore the application effect of nurse led normal all-round management mode in the interruption of nursing in urology department.Methods A nursing interruption management team was set up to collect the data of nursing interruption events,analyze the causes,formulate and implement a normalized all-round management plan to manage the nursing interruption events in the urology department,count the source,type and frequency of the nursing interruption events,and compare the outcome of the interruption events before and after the intervention and the duration of the interruption events.Results From October 2021 to September 2022,4122 NI occurred and were observed in 510 hours,with an average of 8.08 times/h.The frequency of interruption events after intervention was significantly lower than that before intervention(P<0.05).Nursing interruption events mainly came from patients'family members(1638%),patients(15.48%),nurses'colleagues(14.87%)and environment(13.05%).The types of nursing interruptions were intrusive(47.65%),divergent(25.62%),destructive(15.02%)and conflicting(11.72%).Compared with before intervention,the negative outcome events after intervention were significantly reduced(P<0.05).The duration of interruption events after intervention was significantly shorter than that before intervention(P<0.05).Conelusion The nurse led normal all-round management model can effectively reduce the interruption of urological nursing,improve the outcome of the interruption,and shorten the duration of the interruption.
