芪苈强心胶囊联合沙库巴曲缬沙坦钠治疗阳虚水泛型重症心力衰竭的疗效及心肌保护作用

Efficacy and myocardial protection of Qiliqiangxin Capsule combined with Shakubaqu valsartan sodium for treatment of Yang-deficient water-induced severe heart failure

目的对阳虚水泛型重症心力衰竭患者进行芪苈强心胶囊联合沙库巴曲缬沙坦钠治疗,观察该联合治疗的疗效及心肌保护作用。方法选取江西省玉山县黄家驷医院90例阳虚水泛型重症心力衰竭患者,收治时间为2020年6月-2022年6月,依据入院时间进行编号,应用随机数字表法进行分组,分为常规组及联合组,每组各45例。常规组治疗采用沙库巴曲缬沙坦钠,联合组治疗在常规组基础上加用芪苈强心胶囊,观察两组不同治疗方法的疗效;观察两组患者心肌血清因子血清心肌肌钙蛋白(cTnI)、脑钠肽(BNP)水平及同型半胱氨酸(Hcy)水平;观察两组患者心功能指标,指标主要为左房以及右室内径、心脏左室射血分数(LVEF)和心脏左室舒张末径(LVEDd);观察两组患者不良反应发生概率。结果出院前联合组总有效率大大高于常规组,差异有统计学意义(P<0.05);出院前联合组心肌血清因子cTnI、BNP以及Hcy水平显著低于常规组,差异有统计学意义(P<0.05);联合组出院前心脏左房内径、右室内径、LVEDd显著小于常规组,出院前联合组LVEF显著大于常规组,差异有统计学意义(P<0.05)。结论芪苈强心胶囊联合沙库巴曲缬沙坦钠治疗阳虚水泛型重症心力衰竭的疗效更好,心肌保护作用更强。

Objective Qiliqiangxin Capsule combined with Shakubaqu valsartan sodium for treatment of Yang deficiency water severe heart failure,to observe the combined therapeutic effect and myocardial protective effect.Methods A total of 90 patients with yangdeficiency and water-induced generalized severe heart failure were selected from our hospital from June 2020 to June 2022.They were numbered according to the admission time and divided into convnetional group and combined group by random number table method w,ith 45 cases in each group.Conventional group was treated with shkauba triovalsartan sodium,combined group was treated with Qiqliiangxin capsule on the basis of conventional group,and the efficacy o df ifferent treatments were observed;the levels of myocardial esrum factor,serum cardiac troponin(cTnI),brain natriuretic peptide(BNPa)nd homocysteine(Hcy)in two groups were observed.The indexe sof cardiac function in the two groups were observed,which were mainly letf atrial and right ventricular diameter,left ventricular ejeciton fraction(LVEF)and left ventricular end-diastolic diameter(LVEDd).The probability of adverse reactions was observed in the two groups.Results Before discharge,the total effective rate of the combined group was significantly higher than that of the conventional group,the difference was statistically significant (P<0.05).Before discharge,the levels of myocardial serum factosr cTnI,BNP and Hcy in combination group were significantly lower than those in conventional group,with statistical significance (P<0.05).The left atrial diameter,right ventricular diameter and LVEDd of the combined group were significantly lower than those of the conventional group before discharge,andh te LVEF of the combined group was significantly higher than that of th econventional group before discharge,with statistical signifiacnce(P<0.05).Conclusion Qiliqiangxin Capsule combined with shakubaqu valsartan sodium has better efficacy and myocardial protective effect on Yang deficiency water severe heart failure.
