金因肽喷涂预防头颈部恶性肿瘤放疗患者放射性口腔黏膜炎的效果及对表皮生长因子水平的影响

摘要 目的:探究金因肽对头颈部恶性肿瘤放疗患者放射性口腔黏膜炎(Radiation-induced oral mucositis,RIOM)的预防效果,以期为临床防治RIOM提供参考。方法:选取我院2019年8月至2021年10月头颈部恶性肿瘤放疗患者106例,随机分两组,各53例,对照组给予复方氯己定含漱液含漱,观察组给予联合金因肽喷雾,对比两组患者RIOM损伤情况、发生时间及恢复时间、口腔放射性疼痛分级情况、唾液淀粉酶(Salivary amylase,AMS)及表皮生长因子(Epidermal growth factor,EGF)水平,观察两组不良反应发生情况。结果:观察组中、重度RIOM例数明显低于对照组(P<0.05);观察组RIOM发生时间晚于对照组,RIOM恢复时间低于对照组(P<0.05);相较于观察组,对照组患者疼痛程度更严重(P<0.05);观察组唾液淀粉酶(AMS)及表皮生长因子(EGF)水平高于对照组(P<0.05);两组不良反应发生率比较差异无统计学意义。结论:金因肽应用于头颈部恶性肿瘤放疗患者,可降低中重度RIOM的发生率,缩短创面愈合时间,缓解口腔疼痛,提高AMS及EGF水平,安全性好。 Objective:To explore the preventive effect of Jinyin Peptide on Radiation-induced oral mucositis(RIOM)in patients with head and neck malignant tumor radiotherapy,and provide reference for clinical prevention and treatment of RIOM.Methods:A total of 106 patients with head and neck cancer receiving radiotherapy in our hospital from August 2019 to October 2021 were randomly divided into two groups,with 53 cases in each group.The control group was treated with compound chlorhexidine gargle,and the observation group was given combined Jinyin Peptide spray.The RIOM injury conditions,occurrence time,recovery time,oral radiation pain grade,the levels of Salivary amylase(AMS)and Epidermal growth factor(EGF)were compared in the two groups.The occurrence of adverse reactions was observed in the two groups.Results:The number of moderate and severe RIOM cases in observation group was significantly lower than that in control group(P<0.05).The occurrence time of RIOM in the observation group was later than that in the control group,and the recovery time of RIOM was lower than that in the control group(P<0.05).Compared to the observation group,and the oral radiation pain degree in the control group was more serious(P<0.05).The levels of AMS and EGF in observation group were higher than those in control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusions:The application of Jinyin Peptide in radiotherapy for head and neck malignant tumors can reduce the incidence of moderate and severe RIOM,shorten the wound healing time,relieve oral pain,and improve the levels of AMS and EGF,with good safety.
