摘要
目的:探讨血清免疫细胞因子水平与急性冠脉综合征的相关性。方法:选取我院2019年3月至2021年4月收治的57例急性冠脉综合征患者(ACS组)以及48例同期门诊经冠脉造影确诊为稳定型心绞痛患者(SA组)和45例正常者(对照组)作为研究对象,比较三组患者的类胰蛋白酶(Tryptase,TPS)、血清免疫球蛋白E(Immunoglobin E,IgE)和肿瘤坏死因子α(Tumor necrosis factor-α,TNF-α)水平,并对免疫细胞因子水平与冠脉病变程度的相关性进行分析。结果:ACS组TPS、IgE以及TNF-α水平明显高于SA组和对照组(P<0.05),SA组TPS、IgE以及TNF-α水平明显高于对照组(P<0.05);ACS组中ST段抬高型心梗患者TPS、IgE以及TNF-α水平明显高于非ST段抬高型心梗和不稳定型心绞痛患者(P<0.05);57例ACS患者中,轻度狭窄11例(19.30%),中度狭窄27例(47.37%),重度狭窄19例(33.33%);不同冠脉病变程度亚组中,随着狭窄程度增加,患者TPS、IgE以及TNF-α水平逐渐升高(P<0.05)。经Pearson相关分析显示,TPS、IgE及TNF-α分别与Gensini评分呈正相关。结论:TPS、IgE及TNF-α在ACS患者中表达水平明显升高,且与冠脉病变程度呈正相关,三者联合检测可能有助于早期判断冠脉病变的严重程度。
Objective:To study the relevant of the levels of serum immune cytokine and acute coronary syndrome.Methods:57 patients with acute coronary syndrome(ACS),48 cases of patients with stable angina(SA)diagnosed by coronary angiography and 45 cases of normal persons(control group)from March 2019 to April 2021 were selected as the research objects,then compared the levels of tryptase(TPS),serum immunoglobulin E(IgG)and tumor necrosis factor-α(TNF-α)in three groups and the correlation between the level of serum immune cytokine and the lesion degree of the coronary artery was analyzed.Results:TPS,IgE and TNF-αlevels in ACS group were significantly higher than those in SA group and control group(P<0.05).Their levels in SA group were significantly higher than those in control group(P<0.05).In ACS group,Their levels in patients with ST-segment elevation myocardial infarction were significantly higher than those in patients with non-ST-segment elevation myocardial infarction and unstable angina pectoris(P<0.05).Among 57 patients with ACS,11 cases(19.30%)had mild stenosis,27 cases(47.37%)had moderate stenosis,and 19 cases(33.33%)had severe stenosis.The levels of TPS,IgE and TNF-αincreased with the increase of stenosis degree(P<0.05).Pearson correlation analysis showed that TPS,IgE and TNF-αwere positively correlated with Gensini score.Conclusion:The levels of TPS,IgE and TNF-αwere increased significantly in patients with ACS,and their levels were positively associated with the lesions degree of coronary artery in ACS patients,the joint detection may help to determine the severity of coronary artery lesions.
