New Progress in Diagnosis and Treatment of Cierny-Mader Type Ⅳ Chronic Osteomyelitis

Cierny-MaderⅣ型慢性骨髓炎是一种慢性感染性骨组织疾病。由于临床表现往往不典型以及骨感染范围广泛,导致其诊断和治疗具有挑战性。Cierny-MaderⅣ型慢性骨髓炎的诊断是在基于临床表现的基础上结合实验室和影像学检查,通过骨活检和微生物培养以明确诊断。髓内钉术后感染为一种渗透性的、贯穿性的感染,可将其分类至Cierny-MaderⅣ型骨髓炎进行治疗。可先通过整体治疗将患者调整至相对最佳状态,再根据个体化原则选择最佳手术方法进行局部治疗。目前已有多种疗效良好的治疗方法,但都有着各自的局限性,需要进一步的研究探索以取得更好的结果。

Cierny-Mader type Ⅳ chronic osteomyelitis is a chronic infectious bone disease.Diagnosis and treatment are challenging due to the atypical manifestations and the wide range of bone infections.The diagnosis of Cierny-Mader type Ⅳ chronic osteomyelitis is based on clinical manifestations combined with laboratory and imaging examinations,and confirmed by bone biopsy and microbial culture.Infection after intramedullary nail surgery is a penetrating and penetrating infection,which can be classified into Cierny-Mader type Ⅳ osteomyelitis for treatment.The patients should be adjusted to the best condition by systemic treatment first,and then the best surgical method should be selected for local treatment according to the principle of individualization.At present,there are a variety of effective treatment methods,but they all have their own limitations,and further research is needed to achieve better results.
