压力性尿失禁女性患者经会阴二维盆底超声动态观察结果分析

Analysis of Dynamic Observation Results of Perineal Two-Dimensional Pelvic Floor Ultrasound in Female Patients With Stress Urinary Incontinence

目的分析压力性尿失禁(stress urinary incontinence,SUI)女性患者经会阴二维盆底超声动态观察结果。方法抽取厦门长庚医院2020年1月—2022年11月接诊SUI患者100例为研究组,择同期健康体检者100例为对照组,均行会阴二维盆底超声。分析超声检查尿道后倾比例、形成漏斗状比例、膀胱颈到耻骨联合下缘垂直距离、水平距离、尿道膀胱后角以及近段功能尿道长度。结果静息-Valsalva状态转变中,研究组尿道后倾与形成漏斗状比例高于对照组(P<0.05)。静息、Valsalva以及缩肛状态下,两组膀胱颈到耻骨联合下缘垂直距离、水平距离、尿道膀胱后角差异均有统计学意义(P<0.05)。静息状态下两组近段功能尿道长度差异无统计学意义(P>0.05)。Valsalva以及缩肛状态下,研究组近段功能尿道长度均更短(P<0.05)。结论给予压力性尿失禁患者经会阴二维盆底超声,可清晰观察患者不同状态下盆底结构变化情况,与健康者存在明显差别。

Objective To analyze the dynamic observation results of perineal two-dimensional pelvic floor ultrasound in female patients with stress urinary incontinence(SUI).Methods A total of 100 SUI patients admitted to Xiamen Changgung Hospital from January 2020 to November 2022 were selected as the research group,and 100 healthy persons were selected as the control group.All patients underwent perineal two-dimensional pelvic floor ultrasound,and the ultrasonic examination was performed to analyze the urethral retroversion ratio,funnel-shaped ratio,vertical distance from bladder neck to pubic symphysis inferior margin,horizontal distance,posterior angle of urethra and proximal functional urethral length.Results During the rest-Valsalva state transition,the proportion of urethral retroversion and funnel formation in the study group was higher than that in the control group(P<0.05).At rest,Valsalva and anal contraction,there were differences in the vertical distance,horizontal distance and posterior angle of urethra and bladder between the two groups(P<0.05).There was no difference in the length of proximal functional urethra between the two groups at rest(P>0.05).The length of proximal functional urethra in the study group was shorter in Valsalva and anal contraction(P<0.05).Conclusion Two-dimensional perineal pelvic floor ultrasound can clearly observe the changes of pelvic floor structure of SUI patients in different states,which is obviously different from that of healthy patients.
