腺苷脱氨酶联合全血γ干扰素对结核性胸膜炎的诊断效能

摘要 目的探讨腺苷脱氨酶联合全血γ干扰素对结核性胸膜炎的诊断效能。方法收集梅州市人民医院呼吸与危重症医学科2021年1月—2022年9月收治的胸腔积液患者,选取行胸膜活检及全血γ干扰素检查的患者52例,其中结核性胸膜炎组25例,非结核性胸膜炎组27例。同时分别采用比色法、酶联免疫吸附法检测两组胸腔积液腺苷脱氨酶(adenosine deaminase,ADA)水平及全血γ干扰素,并通过绘制ROC曲线评估ADA对结核性胸膜炎的诊断效能。所有数据均采用SPSS 26.0统计软件进行分析。结果胸腔积液ADA在结核性胸膜炎组及非结核性胸膜炎组的水平分别为(50.26±31.24)U/L和(25.47±24.70)U/L,两者之间差异有统计学意义(P<0.05)。当全血γ干扰素联合胸腔积液ADA进行串联合检测时,诊断特异度最高为81.5%。结论联合检测胸腔积液ADA及全血γ干扰素可提高结核性胸膜炎的诊断效能。 Objective To investigate the diagnostic efficacy of adenosine deaminase combined with whole blood gamma interferon in tuberculous pleurisy.Methods A total of 52 patients with pleural effusion admitted to the department of respiratory and critical care medicine of Meizhou People's Hospital from January 2021 to September 2022 were selected for pleural biopsy and whole blood gamma interferon examination,including 25 patients in the tuberculous pleurisy group and 27 patients in the nontuberculous pleurisy group.At the same time,adenosine deaminase(ADA)level and whole blood interferonγin pleural effusion of the two groups were detected by colorimetric method and enzyme linked immunosorbent assay,and the diagnostic efficacy of ADA in tuberculous pleurisy was evaluated by drawing ROC curve.All data were statistically analyzed by SPSS 26.0 software.Results ADA levels of pleural effusion in tuberculous pleurisy group and non-tuberculous pleurisy group were(50.26±31.24)U/L and(25.47±24.70)U/L,respectively,and the difference between them was statistically significant(P<0.05).When blood gammainterferon combined with ADA in tandem test of pleural effusion,the highest diagnostic specificity was 81.5%.Conclusion The combined determination of ADA and gamma interferon in pleural effusion can improve the diagnostic efficiency of tuberculous pleurisy.
