乳腺癌腋窝淋巴结阳性超声引导下细针穿刺与空芯针穿刺对比

Comparison of Ultrasound-Guided Fine Needle Aspiration and Empty Core Needle Aspiration in Axillary Lymph Node Positive Breast Cancer

目的 探讨乳腺癌腋窝淋巴结临床阳性超声引导下细针穿刺与空芯针穿刺对比。方法 选取2020年1月—2021年10月福建省漳州正兴医院收治的83例乳腺癌患者,同时接受细针穿刺和空芯针穿刺活检检查,分析2种穿刺活检的效果、并发症发生率及满意度等。结果 83例乳腺癌患者中,以77例腋窝淋巴结转移、6例无腋窝淋巴结转移作为手术病理结果。细针穿刺活检和空芯针穿刺活检诊断腋窝淋巴结转移的特异性100%,空芯针穿刺活检敏感性更高,但两者差异无统计学意义(P> 0.05)。与空芯针穿刺相比,细针穿刺满意度明显较高(P<0.05)。与空芯针穿刺相比,细针穿刺并发症发生率相对较低(P <0.05)。结论 超声引导下细针穿刺及空芯针穿刺活检均能对腋窝淋巴结转移进行准确诊断,同病理诊断具有较强的一致性,而在对侵袭性等因素进行详细考虑后,可以将细针穿刺活检作为首选方式。

Objective To compare the accuracy of ultrasoundguided fine needle aspiration with core needle biopsy in axillary lymph nodes of breast cancer.Methods A total of 57 patients with breast cancer admitted to Zhangzhou Zhengxing Hospital of Fujian Province from January 2020 to October 2021 were selected for fine needle aspiration and core needle biopsy.The effects,complications and satisfaction of two kinds of biopsy were observed and analyzed.Results In 83 cases of breast cancer,a total of 77 cases with axillary lymph node metastasis and 6 cases without axillary lymph node metastasis were taken as surgical pathological results.The specificity of fine needle puncture biopsy and hollow needle puncture biopsy in the diagnosis of axillary lymph node metastasis was 100%,while the sensitivity of hollow needle puncture biopsy was higher,but there was no statistical significance between them(P>0.05).Compared with hollow needle puncture,the satisfaction of fine needle puncture was significantly higher(P<0.05).Compared with hollow needle puncture,complication rate of fine needle puncture was relatively lower(P<0.05).Conclusion Both ultrasound-guided fine needle puncture and hollow needle puncture biopsy can accurately diagnose axillary lymph node metastasis,showing strong consistency with pathological diagnosis.However,after considering invasion and other factors in detail,fine needle puncture biopsy can be used as the preferred method.
