生长激素宫腔注入联合宫内节育器对中重度宫腔粘连患者宫腔镜冷刀系统手术后再粘连的预防效果

Effect of intrauterine injection of growth hormone combined with intrauterine device on prevention of re-adhesion after hysteroscopic cold knife system operation in patients with moderate and severe intrauterine adhesion

目的研究生长激素宫腔注入联合宫内节育器对中重度宫腔粘连患者宫腔镜冷刀系统手术后再粘连的预防效果。方法选取2020年1月至2022年4月在舟山市妇幼保健院接受治疗的62例中重度宫腔粘连患者作为研究对象,按照治疗方法不同,分为对照组(n=31)与研究组(n=31)。对照组给予生长激素治疗,研究组在对照组基础上联合应用宫内节育器治疗。比较两组手术前与手术3个月后子宫内膜厚度、术后6个月的月经恢复情况,以及术后3个月的宫腔粘连复发情况。结果手术前,两组患者的子宫内膜厚度比较,差异无统计学意义(P>0.05);术后3个月,两组患者的子宫内膜厚度均明显减少且研究组的子宫内膜厚度显著薄于对照组(P<0.05);研究组术后6个月的月经恢复率显著高于对照组(P<0.05),术后3个月的再次粘连总复发率显著低于对照组(P<0.05)。结论生长激素宫腔注入联合宫内节育器对中重度宫腔粘连患者宫腔镜冷刀系统手术后再粘连的预防效果较为显著,值得临床普及应用。

Objective To study the effect of intrauterine injection of growth hormone combined with intrauterine device(IUD)on the prevention of re-adhesion after hysteroscopic cold knife system surgery in patients with moderate to severe intrauterine adhesion.Methods A total of 62 patients with moderate to severe intrauterine adhesions who received treatment in Zhoushan Maternal and Child Health Hospital from January 2020 to April 2022 were selected and divided into study group(n=31)and control group(n=31)according to different treatment methods.The control group was treated with growth hormone,and the study group was treated with intrauterine device on the basis of the control group.Compared the endometrial thickness of the two groups of patients before and three months after surgery,the menstrual recovery of the two groups of patients six months after surgery,and the recurrence of uterine cavity adhesion three months after surgery.Results The endometrial thickness of the patients in the study group was compared with that of the control group before operation,the difference was not statistically significant(P>0.05).Three months after surgery,the endometrial thickness was significantly reduced in both groups,and the endometrial thickness in the study group was significantly thinner than that in the control group(P<0.05).Six months after surgery,the postoperative menstrual recovery rate of the study group was significantly higher than that in the control group(P<0.05),and the total incidence of postoperative re-adhesion in the study group was significantly lower than that in the control group(P<0.05).Conclusion The effect of intrauterine injection of growth hormone combined with intrauterine device on prevention of re-adhesion after hysteroscopic cold knife system operation in patients with moderate and severe intrauterine adhesion is significant and worthy of popular clinical application.
