摘要
目的探讨核转录因子-κB1(nuclear transcription factor-κB1,NF-κB1)基因rs28362491位点多态性与冠状动脉粥样硬化性心脏病(coronary atherosclerotic heart disease,CHD)患者的严重程度、炎症因子的相关性。方法前瞻性选取2018年6月至2021年6月浙江省金华市人民医院CHD患者173例为观察组,同期非CHD的患者107例为对照组,采用聚合酶链反应–限制性片段长度多态性技术分析NF-κB1基因rs28362491位点基因多态性,检测肿瘤坏死因子-α(tumor necrosis factor–α,TNF-α)、白细胞介素(interleukin,IL)-6、IL-8水平。结果观察组NF-κB1基因rs28362491位点DD基因型频率高于对照组,Ⅱ、ID基因型频率低于对照组(P<0.05)。两组D、I等位基因频率比较,差异有统计学意义(P<0.05)。观察组中,DD基因型组Gensini评分均高于ID和Ⅱ基因型组(P<0.05);ID组和Ⅱ基因型组病变支数、Gensini评分比较,差异无统计学意义(P>0.05)。观察组血清TNF-α、IL-6、IL-8水平显著高于对照组(P<0.05)。观察组DD基因型TNF-α、IL-6、IL-8水平明显高于ID和Ⅱ基因型,差异有统计学意义(P<0.05);ID组和Ⅱ基因型组TNF-α、IL-6、IL-8水平比较,差异无统计学意义(P>0.05)。结论NF-κB1基因rs28362491位点多态性与CHD严重程度及炎症因子相关,DD基因型可能上调TNF-α、IL-6、IL-8水平,成为预测CHD病变程度的危险因子。
Objective To investigate the correlation of rs28362491 locus polymorphism of nuclear factor-KappaB1(NF-κB1)gene with the severity of coronary heart disease(CHD)and inflammatory factors.Methods A total of 173 patients with CHD hospitalized in Department of Cardiovascular,Jinhua People’s Hospital of Zhejiang Province from June 2018 to June 2021 were prospectively selected as the observation group,and 107 patients with non CHD diagnosed by coronary angiography in the same period were selected as the control group.Restriction fragment length polymorphism analysis of NF-κB1 gene rs28362491 gene polymorphism,detection of tumor necrosis factor-α(TNF-α),interleukin(IL-6,IL-8)level.Results The frequency of DD genotype at the rs28362491 site of NF-κB1 gene in the observation group was higher than that in the control group,and the frequencies of Ⅱ and ID genotypes were lower than those in the control group(P<0.05).There was a statistically significant difference in the frequencies of D and I alleles between the two groups(P<0.05).In the observation group,the Gensini score in DD genotype group were higher than those in ID and Ⅱ genotype groups(P<0.05);There was no significant difference in the number of diseased branches and Gensini score between the ID group and genotype Ⅱ group(P>0.05).The serum levels of TNF-α,IL-6 and IL-8 in the observation group were significantly higher than those in the control group(P<0.05).The DD genotypes of TNF-α,IL-6 and IL-8 in the observation group,was significantly higher than that of ID and Ⅱ genotypes,and the difference was statistically significant(P<0.05);There was no significant difference in the levels of TNF-α,IL-6,IL-8 between ID group and Ⅱ genotype group(P>0.05).Conclusion The rs28362491 locus polymorphism of NF-κB1 gene is associated with the severity of coronary heart disease and inflammatory factors.DD genotype may up-regulate the levels of TNF-α,IL-6 and IL-8,and become risk factors for predicting the degree of CHD lesions.
