36例复发性视网膜脱离再手术分析

Clinical analysis of 36 recurrent retinal detachments

目的 :探讨视网膜脱离术后复发原因及再手术效果。方法 :对 3 6例复发性视网膜脱离患者进行分析 ,寻找复发原因 ,并根据复发原因进行再次手术。结果 :术后复发原因包括原裂孔未闭 9眼 ;新裂孔形成 8眼 ;玻璃体增殖 18眼 ;玻璃体积血 1眼。经再次手术后视网膜复位 2 9眼 ,未复位 7眼。结论 :玻璃体增殖是视网膜脱离术后复发的主要原因。术前正确评定玻璃体状态及可能的发展方向 ,对选择正确术式具有重要意义。

Objective:To explore the causes of recurrent retinal detachment and the effects of re-operation. Methods:36 cases of recurrent retinal detachment were analyzed.All cases received re-operation according to the causes of recurrence. Results:The causes of recurrence included reopen of perforation in 9 eyes,new perforation formation in 8 eyes,vitreous proliferation in 18 eyes,vitreous hemorrhage in 1 eye.Retinal reattached in 29 eyes after re-operation,7 eyes were not successful. Conclusions:Vitreous proliferation is the main cause of recurrent retinal detachment.It is important to evaluate the vitreous status and it′s developmental direction before re-operation for selecting correct surgical procedure.
