云南省口腔护理人力资源配置及公平性分析

Analysis on Human Resource Allocation and Equity of Oral Care in Yunnan Province

目的:对云南省口腔护理人力资源的配置现状及公平性进行分析,为政府卫生资源配置提供根据。方法:描述性分析“十三五”末云南省口腔护理人力资源配置现状,应用基尼系数、洛伦兹曲线从人口、地理面积和经济3个方面对护理人力资源配置状况进行公平性分析;用泰尔指数计算地区间和地区内差异对口腔护理人力资源配置不平衡的贡献率。结果:云南省口腔护理总人数为6906人,按地理面积、常住人口、经济分布计算的基尼系数分别为0.5920、0.4210、0.2068;按人口和地理分布的总泰尔指数分为0.1373、0.2811,按常住人口和地理分布的区间泰尔指数贡献率分别为67.01%、54.33%,区内贡献率分别为32.99%、45.67%。结论:云南省口腔护理人力资源配置经济分布公平性高于人口分布公平性,人口分布公平性高于地理分布公平性,口腔护理人力资源按经济和人口分布均处于较公平状态,按地理分布总体处于不公平状态,且区间差异大于区内差异。政府及医疗卫生主管部门应进一步优化云南省口腔护理人力资源布局,平衡区域之间的差距,提高资源配置的公平性。

OBJECTIVE To analyze the distribution status and fairness of oral nursing human resources in Yunnan province,and to provide theoretical basis of health resource allocation for the government.METHODS Descriptive research was used to analyze the current situation of human resources allocation of oral care in Yunnan province in 2020,and Gini coefficient and Lorenz curve were used to analyze its fairness from three aspects of population,geographical area and economy.Taier index was used to calculate the contribution rate of regional and intra regional differences to the imbalance of oral care human resources allocation.RESULTS The total number of dental nurse in Yunnan Province was 6906,and the Gini coefficient of the number of oral nursing personnel calculated by geographical area,resident population and economic distribution was 0.5920,0.4210 and 0.2068 respectively.The total Tel index according to population and geographical distribution was divided into 0.1373 and 0.2811.The contribution rate of the interval Tel index according to population and geographical distribution was 67.01%and 54.33%,respectively,and the contribution rate within the region was 32.99%and 45.67%,respectively.CONCLUSION The fairness of oral nursing manpower allocation in Yunnan province was economic distribution population distribution geographical distribution,which was relatively fair according to economic distribution and unfair according to geographical distribution,and the interval difference was greater than the regional difference.The government and medical and health authorities should further optimize the distribution of oral care human resources in Yunnan province,balance the gap between regions,and improve the fairness of resource allocation.
