训练性腹股沟疝的诊断与治疗研究进展

Research Progress in the Diagnosis and Treatment of Training Inguinal Hernia

高强度军事训练可引起各种体外疝,其中训练性腹股沟疝最常见,主要包括传统腹股沟疝、腹股沟脂肪疝、脐内侧襞腹外疝3大类,无论是单发性疝或是复合性疝,均需与腹股沟破裂鉴别,审慎选择治疗方案。腹腔镜技术具有诊断与治疗的双重优势,显著提高了训练性腹股沟疝的诊断与治疗水平。手术策略主要针对内环口、腹股沟管和肌耻骨孔,根据病情不同,优选腹腔镜手术,次选开放手术;对有生育需求者宜选择免补片疝修补,对无生育需求者优选放置聚丙烯补片的疝修补。本文综述了训练性腹股沟疝的诊断与治疗研究进展。

Intense military training can cause a variety of external hernias.As the most common,training inguinal hernia mainly includes three types:traditional inguinal hernia,inguinal adipose hernia and abdominal external hernia of medial umbilical fold.Both single hernia and complicated hernia need to be distinguished from inguinal disruption.Therapeutic methods should be chosen carefully.Laparoscopy has the advantages of both diagnosis and treatment,which has sigificantly improving the diagnosis and treatment of training inguinal hernia.The operative strategy mainly focuses on the internal inguinal ring,inguinal canal and myopectineal orifice.According to different conditions,laparoscopic surgery is a carefully considered as first preference,and traditional open surgery as second choice.The mesh-free herniorrhaphy is preferred in patients with reproductive need.The herniorrhaphy with polypropylene mesh is preferred in patients without reproductive need.This article reviews the latest research progress in the diagnosis and treatment of training inguinal hernia.
