血清乳酸脱氢酶、D-二聚体水平与肺炎支原体肺炎患儿病情严重程度及预后的关联性

Correlation of levels of lactate dehydrogenase and D-dimer with disease severity and prognosis in children with Mycoplasma pneumoniae pneumonia

目的分析乳酸脱氢酶(LDH)、D-二聚体(D-D)与肺炎支原体肺炎患儿病情严重程度及预后的关联性。方法抽取2020年7月至2021年12月南阳市第一人民医院收治的肺炎支原体肺炎患儿146例作为观察组,并以1∶1配比抽取同期健康体检儿童146例作为对照组。检测并比较两组儿童血清LDH、D-D水平,比较观察组不同病情严重程度、入院后不同时间、不同病情时期、不同预后情况患儿的LDH、D-D水平,分析LDH、D-D和病情严重程度、预后的关联性。结果观察组LDH、D-D水平高于对照组(P<0.05);观察组入院第2天时LDH、D-D水平高于入院第14天时(P<0.05);观察组重症患儿、急症期患儿、预后不良患儿LDH、D-D水平高于轻症患儿、恢复期患儿、预后良好患儿(P<0.05)。Pearson分析结果显示,LDH、D-D与病情严重程度(r=0.71、0.69)、预后不良(r=0.69、0.68)呈正相关(P<0.05)。结论肺炎支原体肺炎患儿机体LDH、D-D水平较高,LDH、D-D水平随入院时间延长、疾病恢复呈降低趋势,并与病情严重程度、预后密切相关,可作为临床辅助指标评估患儿病情。

Objective To analyze the correlation of lactate dehydrogenase(LDH)and D-dimer(D-D)with disease severity and prognosis of children with Mycoplasma pneumoniae pneumonia.Methods A total of 146 children with Mycoplasma pneumoniae pneumonia treated in Nanyang First People’s Hospital from July 2020 to December 2021 were selected as the observation group,and 146 children who underwent physical examination during the same period were selected as the control group with a 1∶1 ratio to the observation group.The levels of LDH and D-D of the two groups were detected and compared.The levels of LDH and D-D were compared among patients with different severity,at different time after admission,different disease periods and different prognoses in the observation group.The correlation of LDH and D-D with severity and prognosis of the disease was analyzed.Results The levels of LDH and D-D in the observation group were higher than those in the control group(P<0.05).In the observation group,the levels of LDH and D-D on the 2nd day after admission were higher than those on the 14th day after admission(P<0.05).In the observation group,the levels of LDH and D-D in children with severe disease,children in acute stage and children with poor prognosis were higher than those in children with mild disease,children in recovery stage and children with good prognosis,correspondingly(P<0.05).The results of Pearson analysis showed that LDH and D-D were positively correlated to disease severity(r=0.71,0.69;P<0.05)and poor prognosis(r=0.69,0.68;P<0.05).Conclusions The levels of LDH and D-D in children with Mycoplasma pneumoniae pneumonia are relatively high.The levels of LDH and D-D will decrease with the extension of admission time and disease recovery,and they are closely related to the severity and prognosis of the disease.They can be used as clinical auxiliary indexes to evaluate the condition of children.
