血液循环泵联合踝泵运动对股骨颈骨折患者术后深静脉血栓的预防效果

Effect of blood circulation pump combined with ankle pump movement on prevention of deep venous thrombosis in patients with femoral neck fractures after operation

摘要 目的分析血液循环泵联合踝泵运动对股骨颈骨折患者术后深静脉血栓(DVT)的预防效果。方法采用前瞻性随机对照研究,抽取2019年11月至2021年11月郑州市骨科医院收治的96例股骨颈骨折术后患者为研究对象,依据投掷子法分为对照组(48例,接受踝泵运动干预)与观察组(48例,接受血液循环泵联合踝泵运动干预)。两组均干预7 d。观察并比较两组患者住院天数、干预7 d后患肢肿胀程度、干预前及干预7 d后凝血活酶时间(APTT)及凝血酶原时间(PT)。比较两组干预7 d、术后1个月DVT发生情况。结果观察组住院天数短于对照组,且干预7 d后患肢肿胀程度低于对照组(P均<0.05)。干预前,两组APTT、PT比较差异未见统计学意义(P>0.05);干预7 d后,观察组APTT、PT均大于对照组,差异有统计学意义(P均<0.05)。干预7 d后及术后1个月,观察组DVT发生率(0.00%,0/48;4.17%,2/48)均低于对照组(14.58%,7/48;16.67%,8/48),P均<0.05。结论血液循环泵联合踝泵运动可缩短股骨颈骨折术后患者住院天数,改善患肢肿胀程度,提高APTT、PT水平,有效预防DVT的发生。 Objective To analyze the effect of blood circulation pump combined with ankle pump movement on prevention of postoperative deep venous thrombosis(DVT)in patients with femoral neck fractures.Methods A prospective randomized controlled study was conducted.A total of 96 patients with femoral neck fractures after operation treated in Zhengzhou Orthopedics Hospital from November 2019 to November 2021 were selected as the study objects.According to the throwing method,patients were divided into control group(48 cases,treated by ankle pump movement intervention)and observation group(48 cases,treated by blood circulation pump combined with ankle pump movement intervention).Both groups received a total of 7 days of intervention.The length of hospital stay,the degree of swelling of the affected limb after 7 days of intervention,activated partial thromboplastin time(APTT)and prothrombin time(PT)before and after 7 days of intervention were observed and compared between the two groups.The incidence of DVT after 7 days of intervention and 1 month after surgery were compared between the two groups.Results The length of hospital stay in the observation group was shorter than that in the control group,and the degree of swelling of affected limbs after 7 days of intervention of the observation group was lower than that of the control group(all P<0.05).There was no significant difference in APTT and PT between the two groups before intervention(P>0.05).After 7 days of intervention,APTT and PT in the observation group were longer than those in the control group,and the difference was statistically significant(all P<0.05).The incidence of DVT 7 days after intervention and 1 month after surgery in the observation group were 0.00%(0/48)and 4.17%(2/48),respectively,which were lower than the 14.58%(7/48)and 16.67%(8/48)in the control group(all P<0.05).Conclusions The blood circulation pump combined with ankle pump movement can shorten the hospital stay of patients with femoral neck fractures after operation,alleviate the degree of limb swelling,improve the APTT and PT,and effectively prevent the occurrence of DVT.
