奥卡西平+维生素B_6+左乙拉西坦治疗癫痫患儿的效果及对其视频脑电图的影响

Curative effect of oxcarbazepine combined with vitamin B6 and levetiracetam in children with epilepsy and its influences on video EEG

目的 研究奥卡西平+维生素B_6+左乙拉西坦治疗癫痫患儿的效果及对其视频脑电图的影响。方法 选取2019年3月至2021年3月于西安市儿童医院收治的90例癫痫患儿为研究对象,按随机数表法分为对照组和观察组各45例,对照组患儿采用口服奥卡西平+左乙拉西坦治疗,观察组患儿采用口服奥卡西平+维生素B_6+左乙拉西坦治疗,疗程4个月。比较两组患儿治疗后临床疗效,治疗前后的视频脑电图、骨代谢[骨密度(BMD)]、认知功能[语言智商(VIQ)、操作智商(PIQ)]改善情况以及易怒、激动、攻击性行为、兴奋等不良反应发生率。结果 治疗4个月后,观察组患儿的总有效率为88.89%,明显高于对照组的71.11%,差异有统计学意义(P<0.05);治疗4个月后,观察组患儿的视频脑电图改善总有效率为91.11%,明显高于对照组的73.33%,差异有统计学意义(P<0.05);两组患儿治疗前后不同时间点的BMD水平比较差异均无统计学意义(P>0.05);治疗4个月后,观察组患儿的VIQ、PIQ分别为(98.31±5.06)分、(104.05±5.87)分,明显高于对照组的(96.24±4.33)分、(101.17±6.91)分,差异均有统计学意义(P<0.05);治疗期间,观察组患儿的不良反应总发生率为6.67%,明显低于对照组的22.22%,差异有统计学意义(P<0.05)。结论 OXC、LEV、维生素B_6联用治疗小儿癫痫能有效改善患儿的视频脑电图表现和认知功能,临床应用效果显著,且安全性较高。

Objective To study the curative effect of oxcarbazepine(OXC)combined with vitamin B6 and leve-tiracetam(LEV)in children with epilepsy and its influences on video electroencephalogram(EEG).Methods A total of 90 children with epilepsy admitted to Xi'an Children's Hospital from March 2019 to March 2021 were selected as the study objects.According to random number table method,the patients were divided into a control group and an observa-tion group,with 45 cases in each group.The control group was treated with oral oxcarbazepine+levetiracetam,and the ob-servation group was treated with oral oxcarbazepine+vitamin B6+levetiracetam.The course of treatment lasted 4 months.The clinical efficacy after treatment,improvement of video EEG,bone metabolism[bone mineral density(BMD)],cogni-tive function[language intelligence quotient(VIQ),performance intelligence quotient(PIQ)]before and after treatment,and incidence of adverse reactions(irritability,agitation,aggressive behavior,and excitement)were compared between the two groups.Results After 4 months of treatment,the overall response rate of children in the observation group was significantly higher than that of the control group(88.89%vs 71.11%,P<0.05).After 4 months of treatment,the response rate of video EEG improvement in the observation group was significantly higher than that in the control group(91.11%vs 73.33%,P<0.05).There was no significant difference in BMD between the two groups before and after treatment(P>0.05).After 4 months of treatment,VIQ and PIQ of children in the observation group were(98.31±5.06)points and(104.05±5.87)points,significantly higher than(96.24±4.33)points,(101.17±6.91)points in the control group(P<0.05).The incidence of adverse reactions during treatment in the observation group was significantly lower than that in the con-trol group(6.67%vs 22.22%,P<0.05).Conclusion The combination of OXC,LEV and vitamin B6 in the treatment of pediatric epilepsy can effectively improve the video EEG and cognitive function of children,with significant clinical ap-plication effect and high safety.
