消化内镜治疗应用在老年上消化道非静脉曲张性出血治疗中的临床效果分析

Clinical Effect Analysis of Digestive Endoscopy in the Treatment of El⁃derly Upper Gastrointestinal Non-variceal Bleeding

目的分析探究老年上消化道非静脉曲张性出血患者于治疗期间应用消化内镜的临床效果。方法随机选择2020年1月—2021年12月福建省莆田市第一医院收治的老年上消化道非静脉曲张性出血患者80例作为研究对象,应用随机数表法划分为对照组、观察组,各40例,前者应用常规药物治疗,后者应用消化内镜治疗,对比两组患者临床治疗效果,评估指标包含出血情况、临床治疗有效率、并发症发生率、凝血功能、血清炎性因子。结果两组止血时间、血流速度、出血量、退热时间、呕血消失时间以及大便潜血试验转阴时间对比,差异有统计学意义(P<0.05);对照组患者临床治疗有效率为77.50%低于对照组的95.00%,差异有统计学意义(P<0.05);观察组并发症发生率2.50%较对照组的20.00%低,差异有统计学意义(P<0.05);治疗后,两组凝血功能等相关参数对比,差异有统计学意义(P<0.05);治疗后,两组炎性因子等指标数值对比,差异有统计学意义(P<0.05)。结论老年上消化道非静脉曲张性出血于治疗期间应用消化内镜具有理想效果,具有较高的安全性以及可行性,可降低患者出血量,缩短止血时间,有利于改善机体凝血功能以及血清炎性因子。

Objective To analyze and explore the clinical effect of digestive endoscopy in elderly patients with upper gastrointestinal non-variceal bleeding during treatment.Methods 80 elderly patients with non variceal upper gastroin⁃testinal bleeding admitted to the First Hospital of Putian City,Fujian Province from January 2020 to December 2021 were randomly selected as the research subjects.They were divided into a control group and an observation group us⁃ing a random number table method,with 40 patients in each group.The former received conventional drug treatment,while the latter received digestive endoscopy treatment.The clinical treatment effects of the two groups of patients were compared,with evaluation indicators including bleeding status,clinical treatment efficiency,and incidence of complications,coagulation function and serum inflammatory factors.Results There was a statistically significant differ⁃ence(P<0.05)between the two groups in terms of hemostasis time,blood flow velocity,bleeding volume,fever relief time,emesis disappearance time,and fecal occult blood test negative conversion time(P<0.05).The clinical treatment effectiveness rate of the control group patients was 77.50%,which was lower than 95.00%of the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group 2.50%was lower than 20.00%in the control group,and the difference was statistically significant(P<0.05).After treatment,there was a statistically significant difference in coagulation function and other related parameters between the two groups(P<0.05).After treatment,there was a statistically significant difference in the values of inflammatory factors and other indicators between the two groups(P<0.05).Conclusion The application of digestive endoscopy during the treatment of non varicose upper gastrointestinal bleeding in the elderly has an ideal effect,high safety and feasibility.It can reduce the patient's bleeding volume,shorten the hemostasis time,and improve the body's coagulation function and serum inflammatory factors.
