骨肉瘤患者大剂量甲氨蝶呤化疗和血药浓度监测的护理分析

Nursing Analysis of High Dose Methotrexate Chemotherapy and Blood Concentration Monitoring in Patients with Osteosarcoma

目的探析骨肉瘤患者大剂量甲氨蝶呤化疗和血药浓度监测的护理方法。方法回顾性选取福州市第二医院2020年11月—2021年6月收治的12例骨肉瘤患者的资料进行分析,将其按照入院顺序分为专项护理组、常规护理组,每组6例。所有患者均行为期12周的大剂量甲氨蝶呤化疗,常规护理组行常规护理,专项护理组行大剂量甲氨蝶呤化疗专项护理,在护理结束后分析患者的生活质量变化情况,统计患者的护理满意度,并用焦虑自评量表、抑郁自评量表计算负面情绪评分。结果专项护理组的生活质量评分显著优于常规护理组,差异有统计学意义(P<0.001);专项护理组护理满意度为100.0%(6例),优于常规护理组的50.0%(3例),但差异无统计学意义(P>0.05);专项护理组患者护理后的负面情绪评分显著优于常规护理组,差异有统计学意义(P<0.001)。结论在骨肉瘤患者进行大剂量甲氨蝶呤化疗过程中予以血药浓度监测及专项护理是保证患者生活质量及减少负面情绪的主要环节。

Objective To explore the nursing method of high dose methotrexate chemotherapy and blood drug concen⁃tration monitoring in patients with osteosarcoma.Methods Data of 12 patients with osteosarcoma admitted to Fuzhou Second Hospital from November 2020 to June 2021 were retrospectively analyzed,and they were divided into special nursing group and routine nursing group according to the order of admission,with 6 cases in each group.All patients received 12-week high-dose methotrexate chemotherapy,routine nursing group received routine nursing,special nurs⁃ing group received high-dose methotrexate chemotherapy special nursing.After the end of nursing,the changes in the quality of life of patients were analyzed,the nursing satisfaction of patients was calculated,and the negative emotion score was calculated by self-rating anxiety scale and self-rating depression scale.Results The quality of life score of the special nursing group was significantly better than that of the routine nursing group,the difference was statistically significant(P<0.001).The nursing satisfaction of special nursing group was 100.0%(6 cases),better than that of rou⁃tine nursing group 50.0%(6 cases),but the difference was not statistically significant(P>0.05).The negative emotion score of special nursing group was significantly better than that of routine nursing group(P<0.001).Conclusion Blood concentration monitoring and special nursing during high-dose methotrexate chemotherapy in patients with osteosar⁃coma are important links to ensure the quality of life of patients and reduce negative emotions.
