鼻内窥镜下微型局部填塞治疗鼻腔深部出血疗效分析

Analysis of Curative Effect of Miniature Partial Packing under Nasal Endoscope in the Treatment of Deep Nasal Hemorrhage

目的分析鼻内窥镜下微型局部填塞治疗鼻腔深部出血的疗效。方法随机选取2018年5月—2021年5月在兴化市第二人民医院治疗的鼻腔深部出血患者60例为研究对象,依据抛硬币法将患者分为两组,各30例。对照组予以鼻内窥镜下凡士林纱条和膨胀止血材料局部填塞止血;治疗组予以鼻内窥镜下0.9%氯化钠注射液浸润的明胶海绵和膨胀止血材料微型局部填塞止血,对两组临床疗效、各症状缓解时间、并发症发生情况进行观察比较。结果治疗组临床治愈率(96.67%)明显高于对照组,差异有统计学意义(χ^(2)=4.706,P<0.05)。治疗组鼻塞缓解时间为(2.34±0.77)d,黏膜水肿消散时间为(4.14±1.26)d,鼻涕减少时间为(4.24±1.47)d,头痛缓解时间为(5.12±1.53)d,均明显短于对照组,差异有统计学意义(t=7.868、4.896、2.343、4.072,P<0.05)。治疗组并发症发生率(0.00%)明显低于对照组,差异有统计学意义(χ^(2)=4.630,P<0.05)。结论鼻腔深部出血患者应用鼻内窥镜下微型局部填塞治疗的效果更好,能够进一步加快患者症状缓解,预防并发症的发生。

Objective To study and analyze the curative effect of miniature partial packing under nasal endoscope in the treatment of deep nasal bleeding.Methods Sixty patients with deep nasal bleeding who were treated at the Second People's Hospital of Xinghua City from May 2018 to May 2021 were randomly selected as the research subjects.According to the coin toss method,the patients were divided into two groups,with 30 cases in each group.The control group was treated with local packing of Vaseline gauze and expanded hemostatic material under nasal endoscopy for hemostasis;the treatment group was treated with gelatin sponge infiltrated with 0.9%sodium chloride injection under nasal endoscopy and micro local packing with expanded hemostatic material to stop bleeding.The clinical efficacy,symptom relief time,and incidence of complications were observed and compared between the two groups.Results The clinical cure rate of the treatment group(96.67%)was significantly higher than that of the control group,and the difference was statistically significant(χ^(2)=4.706,P<0.05).The relief time of nasal congestion in the treatment group was(2.34±0.77)d,the dissipation time of mucosal edema was(4.14±1.26)d,the reduction time of nasal mucus was(4.24±1.47)d,and the relief time of headache was(5.12±1.53)d,all of which were significantly shorter than the control group,and the difference was statistically significant(t=7.868,4.896,2.343,4.072,P<0.05).The incidence of complications in the treatment group(0.00%)was significantly lower than that in the control group,and the difference was statistically significant(χ^(2)=4.630,P<0.05).Conclusion The treatment effect of micro-local packing under nasal endoscope is better for patients with deep nasal bleeding,which can further accelerate the relief of symptoms and prevent the occurrence of complications.
