摘要
目的:探讨氯氮平治疗诱导的代谢综合征是否会加重精神分裂症患者的认知功能损害。方法:招募69名长期接受氯氮平单一治疗的精神分裂症患者,根据《美国国家胆固醇教育计划成人治疗组第3次指南》将患者分为合并代谢综合征组和不合并代谢综合征组,并招募48名健康人群作为对照组。对所有人群使用重复性成套神经心理状态测验(RBANS)进行认知功能评估。结果:精神分裂症患者即刻记忆、视觉广度、言语功能、注意、延时记忆量表评分及RBANS总分均明显低于健康人群,差异有统计学意义(F值分别为41.07、51.06、50.17、62.99、61.72、64.56,P值均<0.05)。合并代谢综合征患者组和不合并代谢综合征患者组在阳性量表评分、阴性量表评分、一般精神病理量表评分及阳性与阴性症状评定量表(PANSS)总分差异均无统计学意义(P>0.05);合并代谢综合征患者组的即刻记忆、视觉广度、注意力、延时记忆量表评分及RBANS总分均低于不合并代谢综合征患者组,差异有统计学意义(F值分别为10.27、4.49、7.32、8.79、11.55,P值均<0.05)。结论:氯氮平治疗诱导的代谢综合征可能会加重精神分裂症患者的认知功能损害。
OBJECTIVE Clozapine had a high risk of inducing metabolic syndrome(MS).The study aimed to explore whether clo⁃zapine induced MS would aggravate cognitive dysfunction in patients with schizophrenia.METHODS A sample of 69 schizophrenia patients receiving long-term clozapine monotherapy were divided into two groups,MS and non-MS,based on the diagnostic criteria of THE NATIONAL CHOLESTEROL EDUCATION PROGRAM’S ADULT TREATMENT PANEL III.It also recruited 48 healthy indi⁃viduals for the control group.Cognitive function was evaluated using the Repeatable Battery for the Assessment of Neuropsycho⁃logical Status(RBANS).RESULTS The score of immediate memory subscale,visuospatial subscale,language subscale,attention sub⁃scale,delayed memory subscale and the total score of RBANS in patients with schizophrenia were significantly lower than those in healthy people,and the difference was statistically significant(F=41.07,51.06,50.17,62.99,61.72,64.56,respectively,P<0.05).There was no significant difference in positive symptom subscale,negative symptom subscale,general psychopathology subscale score and the total score of Positive and Negative Syndrome Scale(PANSS)between MS patients and non-MS patients(P>0.05).Patients with MS had lower immediate memory subscale,visuospatial subscale,attention subscale,delayed memory subscale and to⁃tal RBANS score than non-MS patients(F=10.27,4.49,7.32,8.79,11.55,P<0.05).CONCLUSION MS induced by clozapine treat⁃ment may aggravate cognitive impairment in patients with schizophrenia.
