综合康复训练联合穴位按摩干预脑卒中吞咽障碍患者疗效观察

Effect of Comprehensive Rehabilitation Training Combined with Acupoint Massage on Stroke Patients with Dysphagia

目的探究综合基本康复训练(治疗性训练+电磁刺激治疗)联合穴位按摩干预对脑卒中吞咽障碍患者的临床应用。方法选择2020年12月—2021年12月辽宁中医嘉和医院收治的脑卒中引起吞咽障碍患者96例为研究对象,根据随机数字表法随机分为对照组(48例)和观察组(48例)。对照组给予综合基本康复训练,观察组给予综合康复训练联合穴位按摩干预,采用标准吞咽功能评价量表(SSA)、改良版洼田饮水试验(MWST)评判两组患者的临床疗效。结果与治疗前比较,治疗后两组患者SSA评分均降低(P<0.05),观察组SSA评分低于对照组(P<0.05)。与治疗前比较,治疗后两组患者MWST评分均降低(P<0.05),观察组MWST评分低于对照组(P<0.05)。治疗后,观察组总有效率高于对照组(P<0.05)。结论综合康复训练联合穴位按摩干预脑卒中吞咽障碍这一手段安全有效,可以进一步改善患者的吞咽功能,提高生活质量,效果满意,体现了中西医结合治疗脑卒中后吞咽障碍的优势。

Objective To explore the clinical application of comprehensive basic rehabilitation training(therapeutic training+electromagnetic stimulation treatment)combined with acupoint massage intervention on stroke patients with dysphagia.Methods 96 patients with dysphagia caused by stroke admitted to Jiahe hospital of Liaoning university of traditional Chinese medicine from December 2020 to December 2021 were selected as the study subjects,and were randomly divided into a control group(48 cases)and an observation group(48 cases)based on a random number table method.The patients in the control group were given comprehensive basic rehabilitation training,and the patients in the observation group were given comprehensive rehabilitation training combined with acupoint massage intervention.The clinical efficacy of the two groups was evaluated by standard swallowing function assessment(SSA)and modified water swallow test evaluation(MWST).Results Compared with before treatment,the SSA scores of patients in both groups decreased after treatment(P<0.05),while the SSA scores in the observation group were lower than those in the control group(P<0.05).Compared with before treatment,the MWST scores of patients in both groups decreased after treatment(P<0.05),while the MWST scores in the observation group were lower than those in the control group(P<0.05).After treatment,the total effective rate in the observation group was higher than that in the control group(P<0.05).Conclusion Comprehensive rehabilitation training combined with acupoint massage is a safe and effective method to intervene swallowing disorder after stroke,which can further improve the swallowing function of patients,improve the quality of life,and the effect is satisfactory,reflecting the advantages of integrated traditional Chinese and Western medicine in the treatment of swallowing disorder after stroke.
