基于中医传承计算平台探究针刺治疗多囊卵巢综合征选穴组方规律

Research on Rules of Acupoints Selection and Prescription in the Treatment for Polycystic Ovarian Syndrome Based on the Traditional Chinese Medicine Inheritance Computing System

目的基于中医传承计算平台V3.0探究针刺治疗多囊卵巢综合征(PCOS)临床研究文献的选穴规律,为临床处方提供新思路。方法全面检索近20年中国期刊全文数据库(CNKI)、万方(WanFang Data)、维普数据库(VIP)、PubMed中有关针刺治疗PCOS的文献,利用中医传承计算平台(TCMICS)进行腧穴与处方的频次、分类、关联规则、聚类的可视化分析,探析针刺治疗PCOS的选穴与组方规律。结果纳入文献155篇,常用腧穴84个,腧穴选用总频次为1076次。针刺治疗PCOS最常用的5个主穴为三阴交、关元、子宫、中极、气海;特定穴以治疗多条经脉的交会穴,治疗脏腑病变的募穴、背俞穴为主;主穴的分布以腹部、下肢与背部为主;治疗处方多由5个腧穴组成。结论研究发现针刺治疗PCOS多采用任脉、足太阴脾经、足阳明胃经腧穴;所选腧穴以“辨病取穴”和“近部选穴”为主体;三阴交、关元、子宫为针刺治疗PCOS的主要穴位;腧穴配伍重视“上下配穴”和“前后配穴”。

Objective To explore the law of acupoint selection in the clinical research literature of acupuncture treatment of polycystic ovary syndrome(PCOS)and provide new ideas for clinical prescription.Methods Literature on acupuncture in the treatment of PCOS from the CNKI,WanFang Data,VIP,PubMed database nearly two decades,visual analysis frequency,classification,association rules and clustering of the acupoints and prescription by the Traditional Chinese Medicine Inheritance Computing System V3.0(TCMICS),to probe into the rules of acupoints selection and prescription for PCOS.Results A total of 155 literatures were included,84 common acupoints were selected,the total frequency of acupoint selection is 1076 times.The five most commonly used acupuncture points for the treatment of PCOS are Sanyinjiao(SP6),Guanyuan(BL26),Zigong(EX-CA1),Zhongji(RN3)and Qihai(BL24);Specific acupoints are mainly crossing points for treating multiple meridians,Mu acupoints and back-Shu points fot treatment of visceral lesions;the main points are divided into abdomen,lower limbs and back;the prescriptions of acupuncture treatments is mostly composed of five acupoints.Conclusion The study found that conception vessel,spleen meridian of foot-Taiyin,yangming stomach meridian of foot were often used in the treatment of PCOS by acupuncture;The selected acupoints are“disease differentiation-acupoints selection”“selection of local points”as the principal part;Sanyinjiao(SP6),Guanyuan(BL26)and Zigong(EX-CA1)are the main acupuncture points for the treatment of PCOS;Acupoint compatibility pays more attention to local acupoint selection and anterior-posterior point association.
