髋关节置换术联合唑来膦酸治疗老年股骨颈骨折的临床疗效及对骨代谢指标的影响

Clinical efficacy of hip arthroplasty combined with zoledronic acid in the treatment of femoral neck fractures in the elderly patients and the effect on bone metabolic indexes

目的:探讨唑来膦酸对老年股骨颈骨折(FNF)髋关节置换术后患者的临床疗效及对骨代谢指标的影响。方法:将106例老年股骨颈骨折患者随机分为对照组和观察组,各53例。两组均给以髋关节置换术,对照组术后给以西医常规治疗+康复训练,观察组在对照组的基础上给以唑来膦酸治疗,比较两组的临床疗效以及骨代谢指标的改善效果。结果:治疗后,观察组总有效率高于对照组;血清碱性磷酸酶和抗酒石酸酸性磷酸酶5b水平低于对照组;术后6个月和12个月,观察组髋关节功能评分、生活质量评分、骨密度以及血清骨钙素水平高于对照组(P<0.05)。结论:唑来膦酸对FNF髋关节置换术后患者疗效良好,能改善患者的关节功能,提高骨密度,促进骨代谢。

Objective:To explore the clinical efficacy of zoledronic acid and its effect on bone metabolic indexes in elderly patients after hip arthroplasty for femoral neck fracture(FNF).Methods:One hundred and six patients were randomly divided into a control group and an observation group with 53 cases each.Both groups underwent hip arthroplasty.The control group was treated with routine Western medicine+rehabilitation training,while the observation group was treated with zoledronic acid based on the therapy for the control group.The clinical efficacy and bone metabolic indexes were compared between the two groups.Results:The clinical efficacy was higher,the levels of alkaline phosphatase and tartrate-resistant acid phosphatase 5b were lower in the observation group than the control group after the treatment.The scores of hip function and life quality,the bone mineral density and the serum osteocalcin levels 6 and 12 months after surgery were higher in the observation group than the control group(P<0.05).Conclusion:Zoledronic acid has good efficacy on FNF patients after hip arthroplasty,and can improve the joint function,bone density and bone metabolism of the patients.
