摘要
目的:分析高校教职工前列腺特异性抗原(PSA);糖类抗原125(CA125);糖类抗原199W(CA199W);癌胚抗原(CEA);甲胎蛋白(AFP)5种肿瘤标志物检验结果,为疾病预防、早期筛查和诊断提供参考。方法:测定南京航空航天大学4781名教职工血清5种肿瘤标志物表达水平,其中男性2727名(57%),平均年龄(51.38±16.69)岁;女性2054名(43%),平均年龄(48.97±16.56)岁。彩超检查肾脏结石、肾囊肿。心电图检查心脏情况。分析5种肿瘤标志物的年龄和性别的分布以及与心血管疾病和肾脏疾病的关系。结果:CA199W、CEA和AFP与男性和女性的年龄均呈正相关(均P<0.001);PSA与男性的年龄呈正相关(P<0.001),CA125与女性的年龄呈负相关(P<0.001)。CA199W、CEA、AFP升高的检出率分别为4.35%、2.15%、1.69%。男性PSA升高的检出率为5.13%,女性CA125升高的检出率为3.41%。随着年龄的增长,男性PSA、CA199W和CEA升高的检出率均逐渐升高,差异均有统计学意义(均P<0.01)。女性CA125升高检出率有逐渐下降趋势,CA199W和CEA升高检出率则随着年龄的升高而逐渐升高趋势,差异均有统计学意义(均P<0.01),AFP升高检出率在50~60岁年龄段最高,在60~70岁检出率最低,差异有统计学意义(P=0.013)。5种肿瘤标志物水平在心血管疾病患者和非心血管疾病组间的差异均无统计学意义(均P>0.05)。除CA199W外,其余4肿瘤标志物肾脏疾病组的水平均高于非肾脏疾病组,差异均有统计学意义(均P<0.05)。结论:教职工体检肿瘤标志物表达水平与年龄有关;部分与性别有关,PSA、CA125、CEA和AFP水平升高与肾脏结石或肾囊肿有关。肿瘤标志物的升高需结合临床表现、B超等多种检查做出综合判断。
Objective:To analyze the detection results of five tumor markers of prostate specific antigen(PSA);carbohydrate antigen 125(CA125);carbohydrate antigen 199 W(CA199W);carcinoembryonic antigen(CEA);alpha fetoprotein(AFP)in the university staff,and provide reference for early screening and diagnosis of diseases.Methods:The expression levels of serum tumor markers were measured in 4781 university staff in Nanjing Aeronautics and Astronautics University,among them there were 2727 males(57%),and the average age was(51.38±16.69)years;there were 2054 females(43%),and the average age was(48.97±16.56)years.Kidney stones or renal cysts were examined by color Doppler ultrasound.ECG checked the heart condition.The distribution of age and sex and the relationship with cardiovascular disease and kidney disease were analyzed.Results:CA199W,CEA and AFP levels were positively correlated with the age of the males and females(all P<0.001),PSA was positively correlated with the age of the males(P<0.001),CA125 was negatively correlated with the age of the females(P<0.001).The increased detection rates of CA199W,CEA,and AFP were 4.35%,2.15%and 1.69%,respectively.The increased detection rate of PSA in the males was 5.13%,and the increased detection rate of CA125 in the females was 3.41%.With the increase of age,the detection rates of PSA,CA199W and CEA in the males increased gradually,and the difference was statistically significant(all P<0.01).The increased detection rate of CA125 in the females had a gradual downward trend,the detection rate of CA199W and CEA increased gradually with age increasing,and the difference was statistically significant(all P<0.01).The increased detection rate of AFP was the highest in the age range of 50-60 years,and the lowest in the age range of 60-70 years,and the difference was statistically significant(P=0.013).There was no significant difference in the levels of five tumor markers between patients with cardiovascular disease and non-cardiovascular disease groups(both P>0.05).Except CA199W,the levels of the other four tumor markers in the kidney disease group was higher than those in the non-kidney disease group,and the difference was statistically significant(all P<0.05).Conclusion:The expression level of tumor markers is related to age;some of them are related to gender,the elevated levels of PSA,CA125,CEA and AFP are related to kidney stones or renal cysts.The increase of tumor markers should be combined with clinical manifestations,B ultrasound and other examinations to make a comprehensive judgment.
