综合护理对上消化道异物急诊胃镜治疗患者异物取出成功率、心理应激状态的影响

Effects of comprehensive nursing on the success rate of foreign body removal and psychological stress state in patients with upper gastrointestinal foreign body treated by emergency gastroscopy

目的分析综合护理对上消化道异物急诊胃镜治疗患者异物取出成功率、心理应激状态的影响。方法选取2019年5月至2021年5月来我院开展急诊胃镜治疗的106例上消化道异物患者为研究对象,利用抛硬币法将其分为对照组与研究组,各53例。两组患者入院后均开展急诊胃镜治疗。对照组开展常规护理,研究组在对照组基础上开展综合护理。比较两组的异物取出成功率、心理应激状态、生命体征指标、疼痛因子水平及不良反应发生情况。结果研究组的异物取出成功率为96.23%,明显高于对照组的81.13%,差异具有统计学意义(P<0.05)。干预前,两组的躯体化、焦虑以及抑郁评分比较,差异无统计学意义(P>0.05);干预3 d后,两组的躯体化、焦虑以及抑郁评分明显低于干预前,且研究组低于对照组(P<0.05)。术前,两组的心率(HR)、平均动脉压(MAP)以及脉搏血氧饱和度(SpO_(2))比较,差异无统计学意义(P>0.05);术毕,研究组的HR、MAP低于对照组,SpO_(2)高于对照组(P<0.05)。术前,两组的P物质(SP)、前列腺素E2(PGE2)以及环氧化酶-2(COX-2)水平比较,差异无统计学意义(P>0.05);术后3 d,两组的SP、PGE2以及COX-2水平明显低于术前,且研究组低于对照组(P<0.05)。研究组的不良反应总发生率为3.77%,低于对照组的16.98%,差异具有统计学意义(P<0.05)。结论将综合护理用于上消化道异物急诊胃镜治疗患者中,有利于提高异物取出成功率,减轻患者的心理和生理应激反应,降低疼痛程度,值得推广和应用。

Objective To analyze the effects of comprehensive nursing on the success rate of foreign body removal and psychological stress state in patients with upper gastrointestinal foreign body treated by emergency gastroscopy.Methods A total of 106 patients with upper gastrointestinal foreign bodies who underwent emergency gastroscopy in our hospital from May 2019 to May 2021 were selected as the research objects.The patients were divided into control group and study group by coin tossing method,with 53 cases in each group.Emergency gastroscopy was performed in both groups after admission.The control group was carried out routine nursing,and the study group was carried out comprehensive nursing on the basis of the control group.The success rate of foreign body removal,psychological stress state,vital signs indexes,pain factors levels and incidence of adverse reactions were compared between the two groups.Results The success rate of foreign body removal in the study group was 96.23%,which was significantly higher than 81.13%in the control group,and the difference was statistically significant(P<0.05).Before intervention,there were no significant differences in somatization,anxiety and depression scores between the two groups(P>0.05);after 3 d of intervention,the scores of somatization,anxiety and depression in the two groups were significantly lower than those before intervention,and those in the study group were lower than the control group(P<0.05).Before operation,there were no significant differences in heart rate(HR),mean arterial pressure(MAP)and saturation of pulse oximetry(SpO_(2))between the two groups(P>0.05);at the end of operation,the HR and MAP of the study group were lower than those of the control group,and SpO_(2) was higher than that of the control group(P<0.05).Before operation,there were no significant differences in the levels of substance P(SP),prostaglandin E2(PGE2)and cyclooxygenase-2(COX-2)between the two groups(P>0.05);at 3 d after operation,the levels of SP,PGE2 and COX-2 in the two groups were significantly lower than those before operation,and those in the study group were lower than the control group(P<0.05).The total incidence of adverse reactions in the study group was 3.77%,which was lower than 16.98%in the control group,and the difference was statistically significant(P<0.05).Conclusion The comprehensive nursing applied in patients with upper gastrointestinal foreign body treated by emergency gastroscopy is beneficial for improving the success rate of foreign body removal,alleviating patients'psychological and physiological stress reaction,reducing pain levels.It is worthy of promotion and application.
