基于循证理念的个性化护理对乳腺切除术患者情绪状态及遵医行为的影响

Effects of individualized nursing based on evidence-based concept on emotional state and compliance behavior of patients undergoing mastectomy

目的分析基于循证理念的个性化护理对乳腺切除术患者情绪状态及遵医行为的影响。方法选取我院2019年1月至2021年1月收治的108例行乳腺切除术的乳腺癌患者为研究对象,根据随机数字表法将其分为对照组和观察组,各54例。对照组开展常规护理,观察组开展基于循证理念的个性化护理。比较两组的手术相关指标、情绪状态、遵医行为及并发症发生情况。结果观察组的术中出血量、总引流量少于对照组,引流管留置时间、切口愈合时间、住院时间均短于对照组,差异具有统计学意义(P<0.05)。干预前,两组的抑郁自评量表(SDS)、焦虑自评量表(SAS)评分比较,差异无统计学意义(P>0.05);出院前,两组的SDS、SAS评分均显著低于干预前,且观察组低于对照组,差异具有统计学意义(P<0.05)。观察组的总遵医率高于对照组,差异具有统计学意义(P<0.05)。观察组的术后并发症总发生率明显低于对照组,差异具有统计学意义(P<0.05)。结论基于循证理念的个性化护理可改善乳腺切除术患者的手术相关指标,减轻负性情绪,提高遵医行为,减少术后并发症,具有重要的临床应用价值。

Objective To analyze the effects of individualized nursing based on evidence-based concept on emotional state and compliance behavior of patients undergoing mastectomy.Methods A total of 108 patients with breast cancer who underwent mastectomy in our hospital from January 2019 to January 2021 were selected as the research objects.According to the random number table method,the patients were divided into control group and observation group,with 54 cases in each group.The control group carried out routine nursing,and the observation group carried out individualized nursing based on evidence-based concept.The operation related indexes,emotional state,compliance behavior and occurrence of complications were compared between the two groups.Results The intraoperative blood loss and total drainage volume of the observation group were less than those of the control group,the drainage tube indwelling time,incision healing time and hospitalization time were shorter than those of the control group,and the differences were statistically significant(P<0.05).Before intervention,there were no significant differences in Self-Rating Depression Scale(SDS)and Self-Rating Anxiety Scale(SAS)scores between the two groups(P>0.05);before discharge,the SDS and SAS scores of the two groups were significantly lower than those before intervention,those of the observation group were lower than the control group,and the differences were statistically significant(P<0.05).The total compliance rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The individualized nursing based on evidence-based concept can improve operation related indexes,reduce negative emotions,improve compliance behavior,and reduce postoperative complications in patients undergoing mastectomy.It has important clinical application value.
