IgM抗体在呼吸道常见病原体感染中的表达及临床意义

摘要 目的探讨Ig M抗体在呼吸道常见病原体感染中的表达及临床意义。方法选取2019年1月—2020年12月赣州市南康区第二人民医院收治的186例呼吸道感染患者为研究对象,采用间接免疫荧光法对肺炎支原体(MP)、肺炎衣原体(CP)、腺病毒(ADV)、呼吸道合胞病毒(RSV)、柯萨奇病毒B等5种呼吸道感染病原体进行lg M抗体检测。观察分析呼吸道常见病原体的检测结果、多重感染情况,不同年龄段及不同季节的检测情况。结果在186例呼吸道感染患者中阳性48例,占比25.81%,其中病原体MP的阳性感染率最高,占比17.74%,病原体CP、R SV、ADV以及柯萨奇病毒B的阳性感染率分别为2.15%,1.61%,2.69%,1.61%;单一病原体感染患者25例,占比13.44%,构成比为52.08%,两种以上病原体感染患者共有23例,总占比为12.37%,总构成比为47.91%;<18岁患者的阳性率为28.07%,18~44岁患者的阳性率为28.89%,45~60岁患者的阳性率为30.19%,>60岁患者的阳性率为19.05%,>60岁患者的阳性率明显较其他组低;春季患者的阳性率为30.61%,夏季患者的阳性率为18.42%,秋季患者的阳性率为19.56%,冬季患者的阳性率为32.08%,冬春季的阳性率比夏秋季高。结论对呼吸道常见病原体进行Ig M抗体检测,有利于判断患者感染病原体的种类和感染类型,为早期合理治疗提供科学依据。 Objective To investigate the expression and clinical significance of IgM antibody detection in common respiratory tract infections.Methods Taking 186 patients with respiratory infections admitted to our hospital from January 2019 to December 2020 as the research objects,the indirect immunofluorescence method was used to detect Mycoplasma pneumoniae(MP),Chlamydia pneumoniae(CP),adenovirus(ADV),and respiratory tract infections.5 kinds of respiratory tract infection pathogens including RSV(RSV)and Coxsackie virus B are tested for IgM antibodies.Observe and analyze the detection results of common respiratory pathogens,multiple infections,detections of different age groups,and detections of different seasons.Results Among the 186 patients with respiratory infections,the total number of positive cases was 48,accounting for 25.81%.The positive infection rate of the pathogen MP was the highest,accounting for 17.74%,and the positive infection rate of the pathogens CP,RSV,ADV and Coxsackie virus B Respectively:2.15%,1.61%,2.69%,1.61%;25 patients infected by a single pathogen,accounting for 13.44%,constituted 52.08%,and there were 23 patients infected by two or more pathogens,accounting for 12.37%.The total constituent ratio is 47.91%;the positive rate for patients under 18 years old is 28.07%,the positive rate for patients aged 18-44 years is 28.89%,the positive rate for patients aged 45-60 years is 30.19%,and the positive rate for patients over 60 years old The positive rate was 19.05%,and the positive rate of patients>60 years old was significantly lower than that of other groups;the positive rate of spring patients was 30.61%,the positive rate of summer patients was 18.42%,and the positive rate of autumn patients was 19.56%.The positive rate of patients in winter was32.08%,and the positive rate in winter and spring was higher than that in summer and autumn.Conclusion IgM antibody detection for common respiratory tract infections is helpful for judging the types of pathogens and infection types of patients,and provides scientific basis for the reasonable treatment of early respiratory tract infections.
