160例药物性皮炎临床特征及诊治分析

目的探讨药物性皮炎临床特征和诊治特点。方法回顾性收集160例药物性皮炎患者临床资料,分析不同严重程度药物性皮炎患者的临床特点、治疗方法。结果160例药物性皮炎患者轻型药物性皮炎以发疹型、重症药物性皮炎以重症多形红斑型多见,男性患者重症比例更高(P<0.05),年龄与药物性皮炎严重程度之间的相关性无统计学意义(P>0.05),白细胞计数、中性粒细胞百分比及淋巴细胞百分比与药物性皮炎的严重程度之间的关联差异存在统计学意义(P<0.05)。重症药物性皮炎患者因常规糖皮质激素治疗效果欠佳,加用阿达木单抗及静脉注射用免疫球蛋白,有效控制病情。结论需早期识别重症药物性皮炎,常规治疗疗效欠佳时尽早联合用免疫球蛋白、生物制剂、免疫抑制剂等新型治疗方法。

Objective To investigate the clinical features,diagnosis and treatment of dermatitis medicamentosa.Methods The clinical data of 160 patients with dermatitis medicamentosa were retrospectively collected,and the clinical features and treatment methods of dermatitis medicamentosa with different severity were analyzed.Results In 160 patients with dermatitis medicamentosa,the mild dermatitis medicamentosa was mainly exanthematous dermatitis medicamentosa type,and the severe dermatitis medicamentosa was mainly severe erythema multiform drug eruption.The proportion of male patients with severe dermatitis medicamentosa was higher,with statistical significance(P<0.05).The correlation between age and the severity of dermatitis medicamentosa was not statistically significant(P>0.05).There was a statistically significant difference in the correlation between white blood cell count,neutrophil percentage,lymphocyte percentage and the severity of dermatitis medicamentosa(P<0.05).Due to the poor effect of routine corticosteroids treatment on severe dermatitis medicamentosa,the addition of adalimumab and intravenous immunoglobulin could effectively control the disease.Conclusion Early identification of severe dermatitis medicamentosa is needed.When the efficacy of conventional treatment is poor,new treatment methods such as intravenous immunoglobulin,biological agents,immunosuppressive agents can be combined as soon as possible.
