全髋与半髋关节置换术治疗老年股骨颈骨折的有效率对比分析

Comparative Analysis of the Effective Rates of Total Hip Arthroplasty and Hhemi-hipArthroplasty in the Treatment of Femoral Neck Fractures in the Elderly

目的:探讨全髋与半髋关节置换术治疗老年股骨颈骨折的效果。方法:选入我院骨科2019年1月-2019年12月接收的股骨颈骨折患者150例,采用随机数字表法均分为观察组和对照组,每组各75例。对照组行半髋关节置换术治疗,观察组行全髋关节置换术治疗,比较2组并发症发生率、手术指标、Harris-髋关节功能评分、生活质量(SF-36)评分、疼痛(VRS与VAS)评分及2组血清水平测定(CRP、IL-6以及TNF-α)比较。结果:观察组并发症发生率为6.67%,低于对照组的16.00%,组间形成差异性(P<0.05);观察组手术用时长于对照组,拔管用时、完全负重用时以及住院时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);术后1个月和2个月,观察组Harris评分高于对照组,2组比较具有差异性(P<0.05);2组生活质量评分比较,观察组优于对照组(P<0.05);2组疼痛评分比较,观察组均低于对照组(P<0.05);2组血清水平测定(CRP、IL-6以及TNF-α)比较,观察组优于对照组(P<0.05)。结论:全髋关节置换术和半髋关节置换术在近期效果对比上基本一致,但是全髋关节置换术的远期康复情况更明显,治疗效果更佳。

Objective:To investigate the effect of total hip and hemi-hip arthroplasty for the treatment of femoral neck fractures in the elderly.Methods:A total of 150 patients with femoral neck fracture who were admitted to the Department of Orthopedics of our hospital from January 2019 to December 2019 were included in the study.They were equally divided into the research group and the control group using the random number table,with 75 cases in one group.The control group was treated with semi-hip joint replacement and the research group was treated with total hip joint replacement.The complication incidence,operation indexes and Harris-hip joint function score of the two groups were compared.Results:The incidence of complications was 6.67%in the study group and lower than 16.00% in the control group,showing a significant difference between the two groups(P<0.05).The operation time in the research group was longer than that in the control group,the catheter removal time,the complete weight-bearing time,and the hospital stay were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group.The differences were statistically significant(P<0.05).One and two months after surgery,the Harris score of the research group was higher than that of the control group,and there was significant difference between the two groups(P<0.05).Conclusion:The short-term effects of total hip replacement and hemi-hip replacement are basically the same,but the long-term rehabilitation of total hip replacement is more obvious and the treatment effect is better.
