全瓷冠修复联合Cortisomol糊剂根管充填对隐裂性牙髓炎患者牙周状态、口腔炎症的影响

Influence of all-ceramic crown restoration combined with Cortisomol paste root canal filling on periodontal status and oral inflammation in patients with cracked pulpitis

目的 探究根管治疗后全瓷冠+Cortisomol糊剂根管充填治疗隐裂性牙髓炎患者的效果。方法 收集2019年2月至2022年1月平顶山市口腔医院86例隐裂性牙髓炎(86牙)患者,按治疗方案不同分成A组(n=43)、B组(n=43)。两组均予以Cortisomol糊剂根管充填,在此基础上,A组配合全瓷冠修复治疗,B组配合金属烤瓷冠修复治疗。对比两组治疗效果、治疗前后视觉模拟评分法(VAS)评分、咀嚼效率、咬合力、牙周指数[探诊深度(PD)、出血指数(BI)]、龈沟液炎性因子[肿瘤坏死因子-α (TNF-α)、C-反应蛋白(CRP)、白细胞介素-6 (IL-6)]水平。结果 与B组总有效率81.40%(35/43)相比,A组总有效率97.67%(42/43)更高(P<0.05);与B组相比,治疗1 d、3 d、7 d后A组VAS评分更低(P<0.05);与B组相比,治疗3个月、6个月后A组咀嚼效率、咬合力更高(P<0.05);与B组相比,治疗3个月、6个月后A组BI、PD更低(P<0.05);与B组相比,治疗7 d、14 d后A组龈沟液TNF-α、CRP、IL-6水平更低(P<0.05)。结论 与金属烤瓷冠修复+Cortisomol糊剂根管充填治疗隐裂性牙髓炎患者相比,应用全瓷冠修复+Cortisomol糊剂根管充填在提升疗效、减轻疼痛感、提高咀嚼效率、咬合力方面更具优势,且对牙周状态、口腔炎症影响更小。

【Objective】To investigate the effect of all-ceramic crown + Cortisomol paste root canal filling in the treatment of patients with cracked pulpitis after root canal therapy.【Methods】Eighty-six patients with cracked pulpitis(86 teeth) in Pingdingshan Stomatological Hospital(February 2019 to January 2022) were collected and divided into group A(n=43) and group B(n=43) according to different treatment plans.Both groups were treated with Cortisomol paste root canal filling.On this basis,group A was treated with all-ceramic crown restoration group,and B was treated with metal-porcelain crown restoration.The treatment effect,Visual Analogue Scale(VAS) score before and after treatment,masticatory efficiency,bite force,periodontal index [probing depth(PD),bleeding index(BI)],inflammatory factors of gingival crevicular fluid [tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),interleukin-6(IL-6)] levels were compared between the two groups.【Results】 Compared with the total effective rate of group B,which was 81.40%(35/43),the total effective rate of group A was higher [97.67%(42/43)](P<0.05).Compare with group B,the VAS score of group A was lower after 1 day,3 days,and 7 days of treatment(P<0.05).Compared with group B,the chewing efficiency and bite force of group A were higher after 3 months and 6 months of treatment(P<0.05).Compared with group B,BI and PD of group A were lower after 3 months and 6 months of treatment(P<0.05).Compared with group B,the levels of TNF-α,CRP and IL-6 in the gingival crevicular fluid of group A were lower after 7 days and 14 days of treatment(P<0.05).【Conclusion】Compared with metal-porcelain crown restoration + Cortisomol paste root canal filling in the treatment of patients with cracked pulpitis,the application of allceramic crown restoration + Cortisomol paste root canal filling can improve the curative effect,reduce pain,improve chewing efficiency and occlusion.It has more advantages in terms of strength,and has less impact on periodontal status and oral inflammation.
