无痛膀胱镜检查术对男性患者焦虑状态的影响及检查效果

The effect of painless cystoscopy on anxiety state of male patients

目的探讨无痛膀胱镜检查术对男性患者焦虑状态的影响以及其检查效果。方法前瞻性收集本院2020年12月至2021年4月接受膀胱镜检查的79例男性患者的临床资料,根据不同检查方式将患者分为普通膀胱镜检查组(38例)和无痛膀胱镜检查组(41例),收集患者的年龄、麻醉方式、进行膀胱镜检查的原因、是否首次接受膀胱镜检查、术前焦虑评分、术后焦虑评分等临床资料。分析比较两组患者的以上指标是否存在差异以及减轻术后焦虑的影响因素。结果所有患者均顺利完成检查,检查效果满意,均无Clavien-DindoⅡ级及以上并发症发生。与普通膀胱镜检查组相比,无痛膀胱镜检查组患者的术后血压、心率、焦虑评分较低,而检查时长较长(均P<0.05)。普通膀胱镜检查组患者的术后收缩压、舒张压、心率和焦虑评分较术前比较,差异均无统计学意义(均P>0.05),而无痛膀胱镜检查组患者的术后血压、心率和焦虑评分较术前明显下降(均P<0.05)。普通膀胱镜检查组有14例(36.8%)患者术后焦虑评分下降,无痛膀胱镜检查组有26例(63.4%)患者术后焦虑评分下降,两组比较差异有统计学意义(P<0.05)。单因素和多因素logistic回归分析结果显示,无痛膀胱镜检查是患者减轻患者术后焦虑的独立促进因素(P<0.05)。结论无痛膀胱镜检查术可以有效地消除膀胱硬镜检查时的疼痛并减轻检查后患者的焦虑状态,从而增加患者及泌尿外科医师的满意度,尤其在提高膀胱癌患者术后定期膀胱镜复查的依从性方面具有较大临床意义。

Objective To explore the influence of painless cystoscopy on the anxiety state of male patients and its examination effect.Methods The clinical data of 79 male patients undergoing cystoscopy were collected in our hospital from December 2020 to April 2021 prospectively.According to different examination methods,the patients were divided into ordinary cystoscopy group(38 cases)and painless cystoscopy group(41 cases).The patient's age,anesthesia method,reason for cystoscopy,whether to receive cystoscopy for the first time,preoperative anxiety score,postoperative anxiety score,and other general information were collected.The differences in the above indicators between the two groups were analyzed and compared,and the influencing factors to reduce postoperative anxiety were analyzed.Results All patients successfully completed the examination,and the examination results were satisfactory.No Clavien-Dindo gradeⅡor above complications occurred.Compared with the general cystoscopy group,the postoperative blood pressure,heart rate and anxiety score of the patients in the painless cystoscopy group were lower,while the examination time was longer(all P<0.05).The postoperative blood pressure,heart rate and anxiety score of the patients in the general cystoscopy group were significantly different from those before surgery(all P>0.05),while the postoperative blood pressure,heart rate and anxiety score of the patients in the painless cystoscopy group were significantly lower than those before surgery(all P<0.05).There were 14 cases(36.8%)in the general cystoscopy group and 26 cases(63.4%)in the painless cystoscopy group whose postoperative anxiety scores decreased,and the difference was statistically significant(P<0.05).Univariate and multivariate logistic regression analysis showed that painless cystoscopy was an independent promoting factor for patients to reduce postoperative anxiety(P<0.05).Conclusions Painless cystoscopy can effectively eliminate the pain during rigid cystoscopy and reduce the anxiety after the examination,so as to increase the satisfaction of patients and urologists,especially in improving the compliance of bladder cancer patients with regular cystoscopy after surgery.
