红外线照射治疗联合甲钴胺对糖尿病周围神经病变患者神经传导速度及炎性因子的影响 被引量:2

Effects of Infrared Radiation Therapy Combined with Mecobalamin on Nerve Conduction Velocity and Inflammatory Factors in Patients with Diabetes Peripheral Neuropathy

摘要 目的探讨红外线照射治疗联合甲钴胺治疗糖尿病周围神经病变(DPN)的临床效果。方法选取2020年4月至2022年4月于本院收治的82例DPN患者作为研究对象,按随机数表法分为两组,各41例。对照组口服甲钴胺治疗,观察组加用红外线照射治疗,持续8周。比较两组患者临床疗效、神经传导速度、炎性因子水平、密西根糖尿病周围神经(MDNS)评分、多伦多临床评分系统(TCSS)评分及不良反应。结果观察组总有效率较对照组高,治疗后正中神经、腓总神经的SNCV、MNCV为(52.47±5.12)m/s、(54.47±5.28)m/s、(49.58±4.23)m/s、(48.41±4.75)m/s,高于对照组的(46.24±4.82)m/s、(47.74±5.12)m/s、(43.71±4.12)m/s、(43.28±4.61)m/s,白介素-6(IL-6)、白介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平及MDNS、TCSS评分为(20.14±2.28)pg/mL、(23.54±2.41)pg/mL、(30.41±3.12)pg/mL、(11.41±1.28)分、(5.84±1.02)分,均低于对照组的(24.41±2.43)pg/mL、(27.65±2.58)pg/mL、(34.87±3.28)pg/mL、(14.04±1.35)分、(7.15±1.14)分,差异有统计学意义(P<0.05);两组安全性相当。结论红外线照射治疗联合甲钴胺可提高DPN疗效,加快神经传导速度,减轻体内炎症反应及临床症状,其安全可靠。 Objective To investigate the clinical effect of infrared radiation combined with mecobalamin in the treatment of diabetes peripheral neuropathy(DPN).Methods A total of 82 patients with DPN admitted to our hospital from April 2020 to April 2022 were selected and divided into two groups according to the random number table,41 patients in each group.The control group was treated with mecobalamin orally,and the observation group was treated with infrared radiation for 8 weeks.The clinical efficacy,nerve conduction velocity,inflammatory factor level,Michigan diabetes peripheral nerve(MDNS)score,Toronto clinical scoring system(TCSS)score and adverse reactions of the two groups were compared.Results The total effective rate of the observation group was higher than that of the control group.After treatment,SNCV and MNCV of median nerve and common peroneal nerve were(52.47±5.12)m/s,(54.47±5.28)m/s,(49.58±4.23)m/s,(48.41±4.75)m/s,higher than those of the control group(46.24±4.82)m/s,(47.74±5.12)m/s,(43.71±4.12)m/s,(43.28±4.61)m/s,interleukin-6(IL-6),interleukin-1β(IL-1β)、Tumor necrosis factor-α(TNF-α)The levels and MDNS,TCSS scores were(20.14±2.28)pg/mL,(23.54±2.41)pg/mL,(30.41±3.12)pg/mL,(11.41±1.28)points,(5.84±1.02)points,which were lower than those of the control group(24.41±2.43)pg/mL,(27.65±2.58)pg/mL,(34.87±3.28)pg/mL,(14.04±1.35)points,(7.15±1.14)points,with statistical differences(P0.05);The safety of the two groups is equivalent.Conclusion Infrared radiation combined with mecobalamin can improve the efficacy of DPN,accelerate the recovery of nerve conduction velocity,reduce inflammatory reaction and clinical symptoms in vivo,and is safe and reliable.
