脂质水胶体硫酸银敷料联合透明质酸凝胶敷料治疗浅二度烧伤患者的临床效果观察

Observation on the clinical effect of lipid hydrocolloid silver sulfate dressing combined with hyaluronic acid gel dressing in treatment of superficial second degree burn

目的:观察脂质水胶体硫酸银敷料联合透明质酸凝胶敷料治疗浅二度烧伤患者的临床效果。方法:分析2021年2月—2022年9月于江苏省苏北人民医院烧伤整形科治疗的130例浅二度烧伤患者的病历资料,根据入院先后顺序分为对照组(2021年2月—11月)和观察组(2021年12月—2022年9月)各65例。两组患者经常规消毒后进行清创处理,对照组患者采用传统磺胺嘧啶银乳膏覆盖创面,观察组患者使用脂质水胶体硫酸银敷料联合透明质酸凝胶敷料覆盖创面,创面外用无菌纱布覆盖,如创面渗液较多可选用棉垫,外以无菌绷带适当包扎固定。比较两组患者创面愈合时间、创面换药次数,采用视觉模拟量表(VAS)评估烧伤初期(第1~3 d)及烧伤中后期(第7~14 d)疼痛程度,比较两组创面感染、敷料粘连、创面出血发生情况。结果:观察组创面愈合时间为(8.32±1.45)d,对照组创面愈合时间为(13.16±2.28)d,组间比较,差异有统计学意义(t=14.442,P<0.05)。观察组创面换药次数为(5.33±1.19)次,对照组创面换药次数为(8.54±1.83)次,组间比较,差异有统计学意义(t=11.856,P<0.05)。观察组患者烧伤初期及烧伤中后期VAS评分低于对照组,组间比较,差异有统计学意义(P<0.05)。伤后1周,观察组创面感染、敷料粘连、创面出血发生率低于对照组,差异有统计学意义(P<0.05)。结论:脂质水胶体硫酸银敷料联合透明质酸凝胶敷料治疗浅二度烧伤具有促进创面愈合、减少换药次数、减轻患者疼痛,避免敷料粘连及创面出血等优势。

Objective To observe the clinical effect of lipid hydrocolloidal silver sulfate dressing combined with hyaluronic acid gel dressing on superficial second-degree burn.Methods The medical records of 130 patients with superficial second-degree burns treated in the Department of Burn and Plastic Surgery of Northern Jiangsu People’s Hospital of Jiangsu Province from February 2021 to September 2022 were retrospectively analyzed,and the medical records of 130 patients with superficial second-degree burns were divided into control group(February~November 2021)and observation group(December 2021 to September 2022)according to the order of admission,with 65 cases each.The two groups of patients were debridement after routine disinfection,the control group patients used traditional sulfadiazine silver cream to cover the wound,the observation group patients used lipid hydrocolloidal sulfuric acid dressing combined with hyaluronic acid gel dressing to cover the wound,the wound was covered with sterile gauze,if the wound exudate was more,you could choose a cotton pad,and the outside was properly bandaged and fixed.The wound healing time and the number of wound dressing changes were compared between the two groups,and the pain degree in the early stage(day 1~3)and the middle and late burns(day 7~14)of burn injury was evaluated by visual analogue scale(VAS),and the occurrence of wound infection,dressing adhesion and wound bleeding in the two groups was compared.Results The wound healing time in the observation group was(8.32±1.45)d,and the wound healing time in the control group was(13.16±2.28)d,and the difference between the groups was statistically significant(t=14.442,P<0.05).The number of wound dressing changes in the observation group was(5.33±1.19)and the number of wound dressing changes in the control group was(8.54±1.83),and the difference between the groups was statistically significant(t=11.856,P<0.05).The VAS scores in the observation group were lower than those in the control group in the early and middle and late stages of burns,and the differences between the groups were statistically significant(P<0.05).One week after injury,the incidence of wound infection,dressing adhesion and wound bleeding in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Lipid hydrocolloidal silver sulfate dressing combined with hyaluronic acid gel dressing in the treatment of superficial second-degree burns has the advantages of promoting wound healing,reducing dressing times,reducing patients’pain,avoiding dressing adhesion and wound bleeding.
