超声引导下经皮肾活检不同取针数对标本检查满意度与并发症发生率的影响

Effect of Different Needle Numbers on Sample Satisfaction and Complication Rate of Ultrasound-guided Percutaneous Renal Biopsy

目的分析超声引导下经皮肾活检不同取针数对标本检查满意度以及并发症发生率的影响,为临床更好地开展肾活检提供依据。方法将行肾活检术的慢性肾脏疾病患者取2针标本的62例设为2针组,取3针标本的48例设为3针组。行超声引导下经皮肾活检时2针组取2针标本,3针组取3针标本。比较两组肾小球获得数、标本检查满意度以及并发症发生率。结果穿刺获得的肾小球数3针组明显多于2针组,差异有统计学意义(P<0.01)。光镜检查满意度2针组与3针组差异无统计学意义(P>0.05)。免疫荧光检查和电镜检查满意度3针组均明显好于2针组,差异有统计学意义(P<0.05)。并发症发生率2针组和3针组差异无统计学意义(P>0.05)。结论超声引导下经皮肾活检取2针与取3针标本光镜检查满意度均较好,但行免疫荧光检查和电镜检查的满意度取3针要优于取2针,同时并发症发生率并未明显增加。

Objective To analyze the effects of different needle numbers on the satisfaction of specimen examination and the incidence of complications in ultrasound-guided percutaneous renal biopsy,so as to provide a basis for better clinical implementation of renal biopsy.Methods A total of 62 patients with chronic kidney disease who underwent renal biopsy were assigned to the 2-needle group,and 48 patients were assigned to the 3-needle group.Ultrasound-guided percutaneous renal biopsy was performed with two specimens in the 2-needle group and three specimens in the 3-needle group.The number of glomeruli obtained,the satisfaction of specimen examination and the incidence of complications were compared between the two groups.Result The number of glomeruli obtained by puncture in the 3-needle group was significantly higher than that in the 2-needle group,and the difference was statistically significant(P<0.01).There was no significant difference in the satisfaction of light microscopy between the 2-needle group and the 3-needle group(P>0.05).The satisfaction of immunofluorescence and electron microscopy in the 3-needle group was significantly better than that in the 2-needle group,and the difference was statistically significant(P<0.05).There was no significant difference in complication rate between the 2-and 3-needle groups(P>0.05).Conclusion Ultrasound-guided percutaneous renal biopsy with 2 needles and 3 needles has good satisfaction with light microscopy,but the satisfaction with immunofluorescence and electron microscopy with 3 needles is better than that with 2 needles,and the incidence of complications is not significantly increased.
