人血管扩张刺激磷蛋白、对氧磷酶1联合检测对氯吡格雷疗效监测的诊断价值研究

Diagnostic value of combined detection of human vasodilator stimulating phosphoprotein and paraoxonase 1 in monitoring the efficacy of clopidogre

目的:探讨人血管扩张刺激磷蛋白(VASP)、对氧磷酶1(PON-1)联合检测对氯吡格雷疗效监测的诊断价值。方法:随机选择2021年10月-2022年9月佛山复星禅诚医院就诊的服用氯吡格雷患者130例为研究对象,采用影像学、临床症状作为氯吡格雷有效性的确认标准,分为反应较差组、反应良好组2组。所有研究对象均检测人血管扩张刺激磷蛋白(VASP)、对氧磷酶1(PON-1)。采用SPSS25.0进行统计处理,采用二元Logistic回归分析检验模型似然比和模型系数,同时构建人血管扩张刺激磷蛋白(VASP)、对氧磷酶1(PON-1)检测及联合检测对结石诊断价值评价的ROC曲线。结果:反应较差组的人血管扩张刺激磷蛋白(VASP)RPI值明显高于反应良好组(χ^(2)=99.526,P<0.01);两组对氧磷酶1无统计学显著差异。ROC曲线表明,人血管扩张刺激磷蛋白(VASP)、对氧磷酶1联合检测ROC曲线下面积(the area under the ROC curve,AUC)为0.951。结论:人血管扩张刺激磷蛋白(VASP)可以作为氯吡格雷疗效监测很好的指标,但对氧磷酶1与氯吡格雷有效性不存在明显相关性,但人血管扩张刺激磷蛋白(VASP)、对氧磷酶1(PON-1)联合检测对氯吡格雷的疗效监测具有很好的诊断价值。

Objective:To investigate the diagnostic value of combined detection of human vasodilator stimulating phosphoprotein(VASP)and paraoxonase 1(PON-1)in monitoring the efficacy of clopidogrel.Methods:130 patients who took clopidogrel from October 2021 to September 2022 in Foshan Fuxing Chancheng Hospital were randomly selected as the study objects.Imaging and clinical symptoms were used as the confirmation criteria for the effectiveness of clopidogrel,and they were divided into two groups:poor response group and good response group.VASP and PON-1 were detected in all subjects.SPSS 25.0 was used for statistical processing,and binary logistic regression analysis was used to test the likelihood ratio and model coefficient of the model.At the same time,the ROC curve for the evaluation of the diagnostic value of human vasodilator stimulated phosphoprotein(VASP),paraoxonase 1(PON-1)detection and joint detection was constructed.Results:The RPI value of vasodilator stimulated phosphoprotein(VASP)in poor response group was significantly higher than that in good response group(χ^(2)=99.526,P<0.01);There was no statistically significant difference in paraoxonase 1 between the two groups.The ROC curve showed that the area under the ROC curve(AUC)measured by VASP and paraoxonase 1 was 0.951.Conclusion:Vasodilatory stimulating phosphoprotein(VASP)can be used as a good indicator for monitoring the efficacy of clopidogrel,but paraoxonase 1 and the efficacy of clopidogrel have no significant correlation,but the combined detection of VASP and paraoxonase 1(PON-1)has a good diagnostic value for monitoring the efficacy of clopidogrel.
