不同剂量猪肺磷脂辅助干预RDS早产儿的效果

The effect of different doses of porcine lung phospholipids in adjuvant intervention in premature infants with RDS

目的研究不同剂量猪肺磷脂辅助干预急性呼吸窘迫综合征(RDS)早产儿的效果。方法选取2020年5月~2022年5月收治的RDS早产儿196例,按照随机数字表法将其分为对照组(98例)、研究组(98例)。两组患儿均予以无创辅助通气干预。在此基础上,对照组予以100 mg/kg的低剂量猪肺磷脂气管内滴入;研究组予以200 mg/kg的高剂量猪肺磷脂气管内滴入。观察两组的肺表面活性物质血清指标、动脉血气分析指标、呼吸力学指标、不良事件发生率。结果与对照组相比,干预后研究组的SP-D、AQP-5水平更高(P<0.05)。与对照组相比,干预后研究组的PaCO_(2)更低(P<0.05);研究组的PaO_(2)更高(P<0.05)。与对照组相比,干预后研究组的IR更低(P<0.05);研究组的CL、FiO_(2)更高(P<0.05)。两组的总不良事件发生率差异无统计学意义(45.92%vs.48.98%,P>0.05)。结论200 mg/kg的高剂量猪肺磷脂辅助干预RDS早产儿,可提高患儿肺表面活性物质血清含量,改善患儿血气指标及呼吸力学指标,且安全性较好。

Objective To study the effect of different doses of porcine lung phospholipids in the adjuvant intervention of premature infants with acute respiratory distress syndrome(RDS).Methods A total of 196 premature infants with RDS admitted to our hospital from May 2020 to May 2022 were divided into the control group and the study group according to the random number table method,each group contained 98 cases.Both groups received non-invasive assisted ventilation intervention.On this basis,the control group was given a low dose of 100 mg/kg porcine lung phospholipid by intratracheal instillation;the study group was given a high dose of 200 mg/kg by intratracheal instillation of porcine lung phospholipid.The serum indexes of pulmonary surfactant,arterial blood gas analysis indexes,respiratory mechanics indexes and the incidence of adverse events in the two groups were observed.Results After intervention,the levels of SP-D and AQP-5 in the study group were higher than those in the control group(P<0.05).After the intervention,the PaCO_(2)of the study group was lower than that of the control group(P<0.05),and the PaO_(2) of the study group was higher than that of the control group(P<0.05).After intervention,the IR of the study groups was lower than that of the control group(P<0.05),and the CL and FiO_(2)of the study group were higher than those of the control group(P<0.05).There was no difference in the incidence of total adverse events between the two groups(45.92%vs.48.98%,P>0.05).Conclusion 200 mg/kg highdose porcine lung phospholipid adjuvant intervention in premature infants with RDS can increase the serum content of pulmonary surfactant,improve blood gas indexes and respiratory mechanics indexes,and has good safety.
