子宫内膜异位症对体外受精或卵胞质内单精子注射-胚胎移植周期胚胎发育及临床结局的影响

Effects of endometriosis on embryo development and clinical outcomes in in vitro fertilization/intracytoplasmic sperm injection-embryo transfer cycles

目的探究子宫内膜异位症(EMs)对行辅助生殖技术助孕后不孕患者胚胎发育及临床结局的影响。方法选取行IVF/ICSI-ET治疗的90例EMs不孕患者及276例输卵管因素不孕患者,回顾性分析2组胚胎发育及临床结局的差异。结果2组的女方年龄、不孕年限、BMI、基础激素浓度、Gn用量、Gn天数及合并男方因素差异均无统计学意义(P>0.05),但EMs组的原发性不孕占比(53.30%)高于对照组(31.50%),差异有统计学意义(P<0.05);EMs组的MⅡ数、卵子成熟率、正常受精数、优质胚胎数、优质胚胎率、有效胚胎数均低于对照组,差异有统计学意义(P<0.05);2组的获卵数、正常受精率、有效胚胎率、优质囊胚率、有效囊胚率及临床结局差异均无统计学意义(P>0.05)。结论EMs对IVF/ICSI-ET后胚胎发育具有负面影响,但对妊娠结局无明显影响。

Objective This paper was aimed to explore the effects of endometriosis(EMs)on embryo development and clinical outcomes of infertile patients undergoing assisted reproduction technology.Methods A total of 90 infertility patients with EMs and 276 infertility patients with tubal factor undergoing IVF/ICSI treatment were selected to retrospectively analyze the differences of embryo development and clinical outcomes between the two groups.Results There was no statistical significance on the differences in female age,duration of infertility,BMI,baseline hormone levels,total dose of Gn,days of Gn duration and associated male infertility between the two groups(P>0.05).However,the odds of primary infertility in EMs group(53.30%)was higher than the control group(31.50%),and the differences were statistically significant(P<0.05).The M I number,oocytes maturation rate,normal fertilization oocytes number,top-quality embryos number,top-quality embryos rate and viable embryosnumber in EMs group were lower than those in control group,and the differences were statistically significant(P<0.05).There was no statistical significance on the differences in terms of number of oocytes retrieved,rate of normal fertilization oocytes,rate of viable embryos,rate of top-quality blastocyst,rate of viable blastocyst and clinical outcomes between the two groups(P>0.05).Conclusion EMs has a negative effect on embryo development after IVF/ICSI-ET,but has no significant effect on pregnancy outcome.
