基于数据挖掘探究王兰娣治疗慢性阻塞性肺疾病用药规律

A study on the medication rule of WANG Landi in treating chronic obstructive pulmonary disease based on data mining

目的:探究甘肃省中医院内科主任王兰娣运用中医药治疗慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)组方的用药规律。方法:收集整理王兰娣的门诊病历,检索其中记录完整的COPD处方,录入Excel 2019表格建立数据库;运用IBM SPSS Statistics 25进行频数分析;运用Origin 2021进行系统聚类分析;运用IBM SPSS Modeler 18.0进行关联规则分析。结果:共筛选处方134首,涉及中药76味。使用频次较高的药物有麻黄、苦杏仁、陈皮、连翘、柴胡、茯苓、干姜、广藿香等;药性以温、寒为主;药味以苦、辛、甘为主;归经以肺经为主;涉及脾、胃等多个脏腑,并得到二阶关联规则药物组合;通过系统聚类分析方法得到聚类6方。结论:王兰娣治疗COPD用药具有寒温同施、培土生金、辛宣苦泄三大特点。

Objective:To investigate the medication rules of WANG Landi's treatment of chronic obstructive pulmonary disease(COPD)in TCM.Methods:WANG Landi’s outpatient medical records were collected,the full COPD prescription was retrieved,and the database was entered into Excel 2019 form to establish a database;IBM SPSS Statistics 25 was used for frequency analysis;Origin 2021 was used for a systematic cluster analysis,and IBM SPSS Modeler 18.0 was used for a correlation rule analysis.Results:A total of 134 prescriptions were screened,involving 76 flavors of TCM medicine.The drugs with high frequency were Mahuang(Herba Ephedrae),Kuxingren(Semen Armeniacae Amarum),Chenpi(Pericarpium Citri Reticulatae),Liaoqiao(Fructus Forsythiae),Chaihu(Radix Bupleuri),Fuling(Poria),Ganjiang(Rhizoma Zingiberis),Guanghuoxiang(Herba Pogostemonis),etc.,and the medicinal properties were mainly warm and cold,and the medicinal taste was mainly bitter,spicy and sweet.Lung meridian is main meridian.Multiple organs such as the spleen and stomach were involved;a combination of second-order related rules drugs was obtained,and 6 clusters were obtained by systematic cluster analysis method.Conclusion:WANG Landi's medication for COPD has three major characteristics:combination of cold drugs and warm drugs in one prescription,tonifying lung with prescriptions for invigorating spleen,and dispersing exterior pathogens with pungent medicine and interior heat with bitter medicine.
