基于数据挖掘探讨肛周脓肿术后疼痛的选穴规律

A study on the acupoint selection rule of treating postoperative pain in perianal abscess based on data mining

目的:以数据挖掘为基础,分析针灸治疗肛周脓肿术后疼痛的选穴规律。方法:以建库至2022年3月为时间段,在中国知网、万方、维普、中国生物医学文献数据库中收集关于肛周脓肿术后针灸的治疗处方。以“肛周脓肿”“术后疼痛”“针灸”“穴位”为主题词进行检索。通过SPSS、 Modeler 18.0、SPSS Statistics 24等软件检索、分析文献中穴位出现的频率以及穴位所属经络。结果:对针灸治疗肛周脓肿术后疼痛的相关文献进行检索归纳,共收录90篇文献,筛选出腧穴处方62个,涉及穴位60个,频次≥7的穴位有10个,分别是承山穴、长强穴、三阴交穴、次髎穴、足三里穴、关元穴、中极穴、二白穴、合谷穴、气海穴,所属经脉以足太阳膀胱经为主。结论:针灸治疗肛周脓肿术后疼痛以近部取穴和循经取穴为主,同时重视特定穴的运用,穴位多属足太阳膀胱经、任脉和督脉,主要集中在下肢部和腰骶部。

Objective:Based on data mining,the selection rules of acupuncture and moxibustion for postoperative pain in perianal abscess patients were analyed.Methods:From the establishment of the databases to March 2022,the treatment prescriptions of acupuncture and moxibustion on perianal abscess surgery were collected from CNKI,Wanfang,VIP,and China Biomedical Literature Database.Searches were conducted with the subject words of perianal abscess,postoperative pain,acupuncture and acupoints.SPSS Modeler 18.0,SPSS Statistics 24 and other software was used to analyze the frequency of acupoints appearing in the retrieved literature and the meridians to which the acupoints belonged.Results:The relevant literature on acupuncture and moxibustion for postoperative pain after perianal abscess was searched and summarized.A total of 90 studies were collected,and 62 acupoint prescriptions were screened out,involving 60 acupoints,and 10 acupoints with a frequency of not less than 7,namely Chengshan(BL57),Changqiang(DU1),Sanyinjiao(SP6),Ciliao(BL32),Zusanli(ST36),Guanyuan(RN4),Zhongji(RN3),Erbai(EX-UE2),Hegu(LI4),Qihai(RN6).The main meridians belong to the foot Taiyang(太阳)bladder meridian.Conclusion:Acupuncture and moxibustion treatment of postoperative pain in perianal abscess patients is mainly based on the selection of acupoints in the proximal part and along the meridian,while paying attention to the use of specific acupoints.Most of the acupoints belong to the foot Taiyang bladder meridian,Ren(任)meridian and Du(督)meridian,and mainly in the lower limbs and waist sacrum.
