自动痔疮套扎术结合外剥内扎术对重度混合痔患者的疗效以及术后并发症的影响

Effect of ruiyun procedure for hemorrhoid combined with external stripping and internal ligation on the treatment of severe mixed hemorrhoids and postoperative complications

目的:探究自动痔疮套扎术(ruiyun procedure for hemorrhoid,RPH)结合外剥内扎术对重度混合痔患者的治疗效果以及对患者术后并发症的影响。方法:于2020年1月至2022年4月,选取110例重度混合痔患者,并采用随机数表法分为观察组(n=55)和对照组(n=55)。其中对照组采用外剥内扎术治疗,观察组采用RPH+外剥内扎术治疗,对比2组疗效、术后疼痛、恢复时间、肛管功能以及并发症。结果:与对照组相比,观察组的治愈率较高,术后12、24、72 h视觉模拟评分法(Visual Analogue Scale,VAS)以及术后并发症发生率更低,手术/住院时间更短(均P<0.05)。观察组术后3个月肛管收缩峰值血流速度(peak systolic velocity,PSV)、舒张末期血流速度(end-diastolic velocity,EDV)均小于对照组(均P<0.05)。结论:RPH结合外剥内扎术可有效提高重度混合痔疗效,降低术后疼痛和并发症风险,促进患者康复。

Objective:To explore the therapeutic effect of ruiyun procedure for hemorrhoid(RPH)combined with external stripping and internal ligation in patients with severe mixed hemorrhoids and its influence on postoperative complications.Methods:From January 2020 to April 2022,110 patients with severe mixed hemorrhoids were selected,and they were divided into an observation group(n=55)and a control group(n=55)by random number table method.The patients in the control group were treated with external stripping and internal ligation,and the observation group was treated with RPH combined with external stripping and internal ligation.The treatment efficacy,postoperative pain,recovery time,canal function,and complications were compared between the 2 groups.Results:Compared with the control group,the patients in the observation group had a higher cure rate,a lower Visual Analogue Scale(VAS)at 12,24,72 h postoperatively,a lower incidence of postoperative complications,and a shorter operation/hospitalization time(all P<0.05).The peak systolic velocity(PSV)and end-diastolic velocity(EDV)of the anal canal in the observation group at 3 months after the operation were lower than those in the control group(all P<0.05).Conclusion:RPH combined with external stripping and internal ligation can effectively improve the therapeutic effect of severe mixed hemorrhoids,reduce postoperative pain and the risk of complications,and promote the recovery of patients.
