加速康复外科理念下延续护理对腹腔镜下子宫切除术患者术后恢复、睡眠质量以及情绪状况的影响

Effect of continuous nursing under the concept of enhanced recovery after surgery on postoperative recovery,sleep quality,and emotional state of patients undergoing laparoscopic hysterectomy

目的:探究加速康复外科理念下延续护理对腹腔镜下子宫切除术患者的术后恢复、睡眠质量以及情绪的影响。方法:选取2020年1月至2022年5月阜阳市人民医院接收的腹腔镜下子宫切除术患者106例,采用随机数表法分为观察组(53例)与对照组(53例)。对照组行加速康复外科理论干预,观察组行加速康复外科理念下延续护理干预。结果:2组患者的术中出血量、并发症总发生率以及手术时间差异均无统计学意义(均P>0.05),观察组患者肠鸣音恢复时间、肛门排气时间、下地活动时间以及住院时间均短于对照组(均P<0.05)。出院前、术后1个月,观察组患者匹兹堡睡眠质量指数得分、汉密尔顿抑郁量表得分均低于对照组(均P<0.05)。结论:相较于加速康复外科理论护理,加速康复外科理念下延续护理可显著促进腹腔镜下子宫切除术患者术后恢复,改善患者出院后的心理状态和睡眠情况,有助于改善患者近中期预后。

Objective:To explore the effects of continuous nursing under the concept of enhanced recovery after surgery on postoperative recovery,sleep quality,and mood of patients undergoing laparoscopic hysterectomy.Methods:From January 2020 to May 2022,106 patients who received laparoscopic hysterectomy in Fuyang People’s Hospital were selected and divided into an observation group(53 patients)and a control group(53 patients)by random number table method.The control group received theoretical intervention of enhanced recovery after surgery,and the observation group received continuous nursing intervention under the concept of enhanced recovery after surgery.Results:There was no difference in intraoperative blood loss,total incidence of complications,and operation time between the 2 groups(P>0.05).The recovery time of bowel sounds,anal exhaust time,ground activity time,and hospital stay in the observation group were shorter than those in the control group(all P<0.05).Before discharge and 1 month after the surgery,the Pittsburgh Sleep Quality Index and Hamilton Depression Scale score of the observation group were lower than those of the control group(both P<0.05).Conclusion:Compared with the theoretical nursing of enhanced recovery after surgery,continuous nursing under the concept of enhanced recovery after surgery can significantly promote the postoperative recovery of patients with laparoscopic hysterectomy,improve the psychological state and sleep condition of patients after discharge,and contribute to the short-term and medium-term prognosis of patients.
