胸主动脉夹层腔内修复术后感染继发主动脉食管瘘1例

Infection complicated with aortic esophageal fistula after transluminal stent-graft placement for aortic dissection:a case report and literature review

主动脉食管瘘(AEF)是胸主动脉腔内修复术后罕见的并发症,因其发病隐匿、死亡率高,给临床诊断和救治带来较大的挑战。AEF的临床表现缺乏特异性,常见表现为发热、呕血、胸背痛、吞咽困难等。本文报道了主动脉夹层支架术后感染继发食管瘘1例,其诊疗过程极为复杂,最终联合心脏外科等多学科后完成救治。现将诊疗过程进行总结,以期提高对AEF的认识,做到早发现、早治疗。

Aortoesophageal fistula(AEF)is a rare complication af ter thoracic endovascular aortic repair(TEVAR).Due to its hidden onset and high mortality,AEF brings great challenges to clinical diagnosis and treatment.The clinical manifestations of AEF are lack of specificity,and the common manifestations are fever,hematemesis,chest and back pain,and dysphagia.This article reported a case of esophageal fistula secondary to infection after aortic dissection stenting.The diagnosis and treatment process was extremely complex,and the treatment was finally completed by the combination of cardiac surgery and other multidisciplinary teams.This article summarizes the diagnosis and treatment process to improve the understanding of AEF and achieve early detection and treatment.
