此文介绍徐莲薇教授治疗原发性痛经的思路及临床经验。原发性痛经是妇科的常见病和多发病。周期性的下腹部疼痛不适大大降低了女性的生活质量。海派陈氏妇科第五代传人徐莲薇教授认为原发性痛经的主要病因病机为气滞血瘀;病位在胞宫、冲任;变化在气血,表现为痛证。陈氏妇科以“病人以元气为本”“女科以血分为要”“妇人以调肝为中心”为主要指导思想,徐师在临证过程中继承发展先师思想,强调从气从血,调周论治原发性痛经;同时重视肾、肝、脾三脏功能;辨别寒热虚实;急则治标,缓则治本;内外合治,不拘一法。在临床上取得了良好疗效,故此文进行整理归纳,并附验案1篇,以飨同道。

This article introduces Professor Xu Lianwei’s ideas and clinical experience in the treatment of primary dysmenorrhea.Primary dysmenorrhea is a common and frequent disease in gynecology.Periodic pain and discomfort in the lower abdomen greatly reduces the quality of life in women.Professor Xu Lianwei,the fifth generation of Haipai Chen’s gynecology,believes that the main cause of primary dysmenorrhea is qi stagnation and blood stasis,the disease is in Chong and Ren,the change is in qi and blood,which is manifested as pain evidence.Chen’s gynecology takes“patients are based on yuan qi”“blood is essential for women”,and“women’s miscellaneous diseases are centered on liver adjustment”as the guiding ideology.In the process of witnessing,Professor Xu inherited and developed the idea of the first teacher,emphasizing that from the perspective of qi and blood,the menstrual cycle should be adjusted to treat primary dysmenorrhea.At the same time,attention should be paid to the functions of the kidney,liver and spleen,the cold and heat should be distinguished,the symptoms should be treated in an urgent manner,and the root cause should be cured if it was not urgent,and the treatment principle of internal and external combination is important.It has achieved good curative effect in clinic,so this paper is sorted out and summarized,and an effective medical records case is attached to share with you.
