耳穴贴压治疗不稳定型心绞痛合并睡眠障碍的临床研究

Clinical Effect of Auricular Acupoint Sticking on Unstable Angina Pectoris Complicated with Sleep Disorders

目的:观察耳穴贴压治疗不稳定型心绞痛(UA)合并睡眠障碍的临床疗效。方法:选取2021年1月—2021年12月河南中医药大学第一附属医院心内科收治的60例UA合并睡眠障碍病人,按照随机数字表法分为对照组和治疗组,每组30例。对照组给予常规治疗,治疗组在治疗UA常规用药基础上加用耳穴贴压方案治疗。比较两组匹兹堡睡眠质量指数(PSQI)、睡眠障碍症状改善疗效、心绞痛疗效积分、中医证候疗效以及不良反应情况。结果:治疗组治疗后PSQI各项评分及总评分均较治疗前明显下降,治疗组治疗后入睡时间、睡眠质量、睡眠障碍、日间功能障碍和总评分低于对照组,差异均有统计学意义(P<0.05或P<0.01)。治疗组和对照组睡眠障碍症状改善疗效总有效率分别为86.67%和56.67%,两组比较差异有统计学意义(P<0.05)。治疗组治疗后疼痛程度、心绞痛持续时间积分和疗效总分低于对照组(P<0.05或P<0.01);治疗组和对照组中医证候疗效总有效率分别为90.00%和63.33%,两组比较差异有统计学意义(P<0.05)。结论:耳穴贴压可以有效改善UA病人的睡眠障碍,改善心绞痛疗效和中医证候积分。

Objective:To analyze the clinical effect of auricular point sticking on unstable angina(UA)patients with sleep disorders.Methods:Sixty patients with UA and sleep disorders were randomly divided into control group and treatment group,with 30 patients in each group.The patients in control group were given routine treatment,and the patients in treatment group were given additional auricular acupoint sticking treatment.The Pittsburgh Sleep Quality Index(PSQI),clinical efficacy of sleep disorders,angina pectoris efficacy score,traditional Chinese medicine(TCM)syndrome,and adverse reactions were compared between two groups.Results:The scores in all dimensions and total score of PSQI in treatment group after treatment were significantly decreased compared with those before treatment,and the sleeping time,sleep quality,sleep efficiency,sleep disorders,hypnotic drugs,daytime dysfunction,and total score in treatment group after treatment were lower than those in control groupe(P<0.05 or P<0.01).The total effective rate of treatment group and control group were 86.67%and 56.67%,respectively,and the difference was statistically significant(P<0.05).After treatment the pain degree,angina duration score,and total curative effect score of treatment group were lower than those of control group(P<0.05 or P<0.01).The total effective rate of TCM syndrome in treatment group and control group were 90.00% and 63.33%,respectively,and the difference was statistically significant(P<0.05).Conclusion:Auricular point sticking can effectively improve sleep disorder of UA patients,the efficacy of angina pectoris,and TCM syndrome score.
