小针刀+推拿治疗急性腰椎间盘突出症的效果刍议

Discussion on the Effect of Small Acupuncture + Tuina in the Treatment of Acute Lumbar Disc Herniation

目的 探究小针刀+推拿治疗急性腰椎间盘突出症的临床效果。方法 随机选取2020年1月—2021年12月期间厦门市中医院收治的80例急性腰椎间盘突出症患者为研究对象,随机分为对照组(推拿治疗,40例)、研究组(小针刀+推拿治疗,40例),比较两组患者的炎症因子水平以及临床症状积分和治疗效果等。结果 研究组治疗总有效率(97.50%)高于对照组(80.00%),差异有统计学意义(χ^(2)=4.507,P<0.05);研究组治疗后的VAS评分、IL-6、IL-1β、TNF-α均低于对照组,JOA、FRS评分均高于对照组,差异有统计学意义(P<0.05);研究组躯体健康、情绪角色等生活质量评分高于对照组,差异有统计学意义(P<0.05);研究组治疗后的趾肌力等症状积分较对照组更低,差异有统计学意义(P<0.05)。结论 小针刀+推拿应用于急性腰椎间盘突出症治疗,对提升治疗效果与生活质量,改善临床症状,降低炎症因子水平,缓解疼痛有重要意义。

Objective To investigate the clinical effect of small acupuncture+tuina in the treatment of acute lumbar disc herniation.Methods 80 patients with acute lumbar disc herniation admitted to Xiamen Traditional Chinese Medicine Hospital from January 2020 to December 2021 were randomly selected as the study subjects.They were randomly divided into a control group(40 cases treated with massage)and a study group(40 cases treated with small needle knife and massage).The inflammatory factors level,clinical symptom scores,and treatment effects of the two groups of patients were compared.Results The total effective rate of 97.50%in the study group was higher than 80.00%in the control group,and the difference was statistically significant(χ^(2)=4.507,P<0.05)Both VAS score,IL-6,IL-1β,TNF-αof the study group after treatment were lower than the control group,and the JOA and FRS scores were higher than the control group,and the difference was statistically significant(P<0.05).The quality of life scores such as physical health and emotional roles in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).The score of toe muscle strength and other symptoms in the study group after treatment was lower compared with the control group,and the difference was statistically significant(P<0.05).Conclusion The application of small needle knife and massage in the treatment of acute lumbar disc herniation is of great significance in improving treatment effectiveness and quality of life,improving clinical symptoms,reducing inflammatory factor levels,and alleviating pain.
