益生菌制剂双歧杆菌联合地奈德乳膏治疗儿童顽固性湿疹的疗效观察

Observation on the Curative Effect of Probiotic Preparation Bifidobacterium Combined with Desonide Cream in the Treatment of Refractory Eczema in Children

目的探讨益生菌制剂双歧杆菌联合地奈德乳膏治疗儿童顽固性湿疹的疗效。方法回顾性分析2020年2月—2022年4月广州市第一人民医院医治的70例顽固性湿疹患儿的病历资料,按照治疗方法分成联合组(35例)、参照组(35例)。比较患儿治疗前、治疗14 d的血清学指标[嗜酸性粒细胞、降钙素原(PCT)]、特应性皮炎疾病评估量表(EASI评分)、治疗效果与复发率。结果治疗14 d后,两组嗜酸性粒细胞和PCT均降低,联合组更低于参照组,差异有统计学意义(P<0.05);两组EASI量表的3个维度(受累面积、皮损严重程度、瘙痒程度)评分均降低,联合组低于参照组,差异有统计学意义(P<0.05);联合组治疗总有效率(91.43%)高于参照组(71.43%),差异有统计学意义(χ^(2)=4.629,P=0.031);随访6个月,联合组复发率(5.71%)与参照组(20.00%)相比,差异无统计学意义(P>0.05)。结论益生菌制剂双歧杆菌联合地奈德乳膏治疗儿童顽固性湿疹,疗效确切,可增强患儿免疫功能,改善皮肤的生理状态,促进皮损相关症状康复。

Objective To investigate the curative effect of probiotic preparation Bifidobacterium combined with desonide cream in the treatment of refractory eczema in children.Methods The medical records of 70 children with intractable eczema treated in Guangzhou First People's Hospital from February 2020 to April 2022 were retrospectively analyzed.According to the treatment,they were divided into combined group(35 cases)and reference group(35 cases).The serological indexes[eosinophils,procalcitonin(PCT)],atopic dermatitis disease assessment scale(EASI score),treatment effect and recurrence rate were compared before treatment and 14 d after treatment.Results After 14 d of treatment,both groups showed a decrease in eosinophils and PCT,and the combined group was even lower than the control group,the difference was statistically significant(P<0.05).The scores of the three dimensions(affected area,severity of skin lesions,and degree of itching)of the two groups of EASI scales decreased,and the combined group was lower than the reference group,the difference was statistically significant(P<0.05).The total effective rate of the combination group(91.43%)was higher than that of the reference group(71.43%),the difference was statistically significant(χ^(2)=4.629,P=0.031);After a follow-up of 6 months,there was no statistically significant difference in the recurrence rate between the combination group(5.71%)and the reference group(20.00%)(P>0.05).Conclusion The probiotic preparation Bifidobacterium combined with desonide cream has a definite curative effect in the treatment of refractory eczema in children.It can enhance the immune function of children,improve the physiological state of the skin,promote the recovery of related symptoms of skin lesions.
