不同严重程度阻塞性睡眠呼吸暂停与代谢综合征的相关性研究

Study on the correlation between obstructive sleep apnea with different severity and metabolic syndrome

目的 探讨不同严重程度阻塞性睡眠呼吸暂停(OSA)与代谢综合征(Mets)的相关性及对心脑血管疾病(CCVD)发生的影响。方法 研究纳入145例疑似阻塞性睡眠呼吸暂停并行多导睡眠监测的患者,依据AHI(睡眠呼吸暂停低通气指数)分为对照组及阻塞性睡眠呼吸暂停轻、中、重度组四个组。比较各组间一般情况、睡眠参数、生化指标及红细胞参数。结果 各组间患者的体质指数(BMI)、收缩压(SBP)、舒张压(DBP)、最长呼吸暂停时间(LAT)、氧减指数(ODI)、最低血氧饱和度(LSpO_(2))、平均血氧饱和度(MSpO2)、甘油三酯(TG)、尿酸(UA)、红细胞(RBC)、血红蛋白(HGB)、血细胞比容(HCT)、高血压患病率、代谢综合征患病率差异有统计学意义(P<0.05)。代谢综合征主要指标中BMI、DBP、TG是影响阻塞性睡眠呼吸暂停严重程度主要指标AHI和ODI的独立危险因素,而BMI、TG是影响LSpO_(2)的独立危险因素(P<0.05)。结论 阻塞性睡眠呼吸暂停的严重程度与代谢综合征的主要指标之间存在着高度的相关性,它们相互影响,可能增加CCVD发生的风险。

Objective To explore the relationship between different severity of obstructive sleep apnea and metabolic syndrome and its effect on cardiovascular and cerebrovascular diseases.Methods 145 patients suspected of OSA and monitored by pornographically were included in the study.They were divided into control group and mild OSA group,moderate OSA group and severe OSA group according to AHI.The general conditions,sleep parameters,biochemical indexes and red blood cell parameters were compared between groups.Results There were significant dfferences in body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),the longest apnea time(LAT),oxygen reduction index(ODI),the lowest(LSPQ)and average oxygen saturation(MSPO_(2)),triglycerides(TG),uric acid(UA),red blood cells(RBC),hemoglobin(HGB),hematocrit(HCT),the prevalence of hypertension and metabolic syndrome among patients in each group(P<0.05).Among the main indicators of Mets BMI,DBP and TG are independent risk factors that affect the main diagnostic indicators of OSA severity AHI and ODI.BMI and TG were independent risk factors for LSpO_(2)(P<0.05).Conclusion There is a high correlation between the severity of OSA and the main indicators of Mets,and their interaction may increase the risk of CCVD.
