老年膝关节骨性关节炎的高频超声与MRI影像特征及诊断价值比较

Comparison of high-frequency ultrasound and MRI imaging features and diagnostic value of elderly knee osteoarthritis

目的 观察老年膝关节骨性关节炎(knee osteoarthritis, KOA)的高频超声与MRI影像的特征,比较其诊断价值。方法 选择我院2020年2月至2022年2月收治的40例老年骨性关节炎患者共58个膝关节进行研究,所有患者均经关节镜检查确诊为KOA,对患者行高频超声检查及MRI检查,比较两种检查的影像学表现及检出率。结果 在KOA的非骨性病变中,滑膜增厚、髌上囊积液、关节软骨退行性变、软骨下骨破坏、半月板病变、髌下脂肪垫损伤、腘窝囊肿、骨质增生及关节周围韧带损伤检出率比较,二者差异无统计学意义(P>0.05),高频超声对关节内韧带损伤检出率低于MRI,对增厚滑膜血流检出率高于MRI,二者差异具有统计学意义(P<0.05)。结论 高频超声和MRI检查在老年膝关节骨性关节炎中具有相近的诊断价值,在关节内韧带、滑膜血管检查中各具优势。高频超声是一种简单价廉的检查方式,可作为膝关节骨性关节炎的首选筛查方法。

Objective To observe the characteristics of high-frequency ultrasound and MRI in elderly knee osteoarthritis(KOA)patients,and to compare their diagnostic value.Methods A total of 58 knee joints of 40 elderly patients with osteoarthritis admitted to our hospital from February 2020 to February 2022 were selected for research.All patients were diagnosed with K0A by arthroscopy,and underwent high-frequency ultrasound and MRI check,observe and compare the imaging performance and detection rate of the two examinations.Results Among the non-osseous lesions of KOA,synovial thickening,suprapatellar sac effusion,articular cartilage degeneration,subchondral bone destruction,meniscal lesions,infrapatellar fat pad damage,popliteal cyst,bone hyperplasia,there was no significant difference in the detection rate of ligament injury around joints between the two groups(P>0.05).the difference was statistically significant(P<0.05).Conclusion High-frequency ultrasound and MRI have similar diagnostic value in elderly knee osteoarthritis,and each has its own advantages in the examination of intra-articular ligaments and synovial blood vessels.Highfrequency ultrasound,as a reloatively simple and cost-effective examination method,can be the preferred screening method for articular osteoarthritis.
